单独应用吲哚菁绿荧光导航技术行乳腺癌前哨淋巴结活检的临床研究

摘　　要：目的 探讨单独应用吲哚菁绿（indocyanine green,ICG）荧光导航技术对乳腺癌患者行前哨淋巴结活检（sentinel lymph node biopsy,SLNB）的可行性. 方法 2012年11月至2014年3月间,136例乳腺癌患者应用ICG荧光导航技术进行SLNB,SLNB结束后行腋窝Ⅰ、Ⅱ水平淋巴清扫以确定腋窝淋巴结状态.结果 SLN检出率为97.1％ （132/136）.在41例腋窝淋巴结转移者中,有2例SLN假阴性,假阴性率为4.9％ （2/41）,敏感性为95.1％（39/41）,预测腋窝淋巴结转移的准确率为98.5％（130/132）.平均每例患者检测出的SLN数量为2.9（1 ～6）枚.结论 ICG荧光导航行SLN检出率高,且具有淋巴管实时显像、有利于寻找SLN的特点.Objective To evaluate indocyanine green （ICG） fluorescence for sentinel lymph node biopsy（SLNB） in patients with breast cancer.Methods From Nov 2012 to Mar 2014,136 breast cancer patients received SLNB using ICG.A level Ⅰ and Ⅱ axillary dissection was performed for verification of axillary node status after the SLNB.Results The SLN identification rate by ICG fluorescence was 97.1％ （132/136）.In 41 cases in whom the axillary lymph node was positive with metastasis,there were two cases that the ICG fluorescence was negative,hence the false-negative rate was 4.9％ （2/41）.The ICG fluorescence sensitivity rate was 95.1％ （39/41） and the accuracy rate was 98.5％ （130/132）.The average number of SLN detected was 2.9 （1-6）.Conclusions Transcutaneous ICG fluorescence imaging for SLNB provides real-time lymphography with high rate of SLN detection and satisfactory accuracy in the detection of axillary metastatic lymph nodes.
