囊袋张力环对合并高度近视的晶状体半脱位患者术后屈光预测准确性的影响

摘　　要：目的观察囊袋张力环(CTR)植入对于合并高度近视的晶状体半脱位患者术后屈光预测的影响。设计回顾性病例系列。研究对象2013年1-12月北京同仁医院眼科白内障中心行白内障超声乳化摘除联合人工晶状体(IOL)植入及CTR植入的合并高度近视的晶状体半脱位患者20例(20眼)。同时期连续行白内障超声乳化未植入CTR的患者20例(20眼)作为对照组。方法回顾患者病历中手术前、后的视力、眼压、内皮细胞计数、屈光状态资料,记录预期屈光度,并计算术后屈光度偏差(术后验光值与术前预留度数的差值)。主要指标预期屈光度、屈光度偏差。结果 CTR植入组和对照组预期屈光度数分别为(-1.29±0.34)D、(-1.16±0.39)D(P=0.31);术后3个月的屈光偏差值分别为(1.38±0.43)D、(1.71±0.32)D(P=0.06)。两组均无术后远视偏差发生。结论囊袋张力环植入对于合并高度近视的晶状体半脱位患者的术后屈光状态无显著性影响,无需调整IOL计算公式和预期度数。Objective To research the influence of capsular tension ring （CTR） on post-operation refraction in lens subluxationpatients combined with high myopia. Design Retrospective case series. Participants 20 patients （20 eyes） with high myopia were per- formed phacoemulcificatioan and IOL implantation as control group. 20 patients （20 eyes） with high myopia were performed phacoemul- cification, IOL and CTR implantation as observed group. All patients came from Beijing Tongren Hospital in Jan. 2013 to Dec. 2013. Method The visual acuity, intraocular pressure, corneal endothelium, and refraction before and after surgery in both groups were re- viewed and compared. Main Outcome Measures Visual acuity, intraocular pressure, coneal endothelium, and refrection predication error. Results Predicative refractive degree in observed group and control group was （-1.29±0.34） D, （-1.16±0.39） D （P=0.31）. Refrac- tive prediction error in observed group and control group in 3 months after phacoemulcification was （1.38±0.43） D, （1.71±0.32） D, respectively （P=0.06）. There was no hyperopia error in two groups after phacoemulcification. Conclusion Capsular tension ring implantation had no significant effect on refractive error in lens subluxation patients combined with high myopia. There is no necessity to additional adiusting IOLcalculating formula in these patients.
