摘 要:目的探讨3.0 T超高场磁共振周围神经全景成像(MRN)对神经纤维瘤病(NF)的临床应用价值。方法搜集疑诊为NF患者34例,均行常规MRI平扫及3D-STIR增强扫描序列,经工作站后处理显像全身神经,评价全身3D-STIR(结合短T1反转恢复时间成像)增强扫描序列对NF的诊断价值。结果结合病理结果及临床诊断,34例患者中NF-Ⅰ型20例,NF-Ⅱ型14例,3.0 T 3D-STIR全身MRN能全面、整体、清晰显示所有患者的全身外周神经根病变及病变与周围神经的相关性,且能显像周围神经远段及较小病灶。结论 3.0 T 3D-STIR全身MRN在NF诊断及病灶检出有重要的临床应用价值。Objective To investigate the clinical value of magnetic resonance neurography (MRN) in diagnosing neurofibromatosis. Methods A total of 34 patients with suspected neurofibromatosis were enrolled in this study. Both routine plain MRI scanning and 3D-STIR sequence scanning were performed in all patients. After post-processing on a workstation, whole body nerves were displayed. The clinical value of enhanced 3D-STIR sequence scanning together with short-T1 inversion recovery time images in diagnosing neurofibromatosis was assessed. Results According to the pathological results and diagnostic criterion established by the National Institute of Health Conference, type I neurofibromatosis was confirmed in 20 cases and type II neurofibromatosis in 14 cases. The whole body enhanced 3D-STIR sequence MRN scanning could completely and clearly display the course, continuity, shape and signal of whole body spinal nerves and related disorders, it could reveal distal peripheral nerves as well as tiny lesions. Conclusion The whole body enhanced 3D-STIR sequence MRN scanning has significant clinical value in detecting and diagnosing neurofibromatosis.
