摘  要:目的分析急性双侧中线旁丘脑梗死综合征的临床和影像表现,加深对该病的认识。方法搜集5例均行CT和MRI检查的急性双侧中线旁丘脑梗死综合征患者资料,分析其CT、MRI和临床表现。结果 CT示1例阴性,1例双侧中线旁丘脑可疑低密度灶,3例双侧中线旁丘脑稍对称性低密度灶。MRI均显示双侧中线旁丘脑较对称分布蝶形长T1、长T2信号,于FLAIR和DWI序列上均为高信号。结论影像学方法,尤其是MRI,对早期诊断和鉴别诊断急性双侧中线旁丘脑梗死综合征有重要临床意义。Objective To investigate the clinical and imaging features of acute bilateral paramedian thalamic infarction syndrome in order to make a further understanding of this disease. Methods Five patients with acute bilateral paramedian thalamic infarction syndrome were collected for this study. Both CT and MRI were performed in all the five patients. The clinical data, CT and MRI manifestations were retrospectively analyzed. Results Plain CT images showed that no acute hemorrhage or infarction was found in 1 case, suspicious bilateral paramedian thalamic infarcts in 1 case ( presenting as suspicious low-density lesions) and bilateral paramedian thalamic infarcts in 3 cases (presenting as somewhat symmetrical low-density lesions). On MR images, symmetrical butterfly-shaped long T1 and long T2 signals could be seen at bilateral paramedian thalamic areas, which were characterized by hyperintensity signals on both FLAIR and DWI sequences. Conclusion Imaging examinations, especially MRI, are very useful in diagnosing and differentiating acute bilateral paramedian thalamic infarction syndrome.
