摘　　要：目的探讨多形性未分化肉瘤的MR表现,提高对该病的诊断水平。方法收集经手术、病理证实的患者11例12个病灶,所有病例均行MR检查。结果 11例多形性未分化肉瘤,大腿6例,小腿1例,上臂1例,肩背部1例,臀部1例,腹膜后1例;4个边缘清楚,8个边缘模糊,周围肌肉软组织均可见不同程度的水肿表现。其中10个信号不均匀,8个T1WI表现为等、低信号,1个病灶内可见少高信号,提示肿瘤内合并出血成分,T2WI抑脂序列所有肿瘤的实质部分均呈不均匀等或稍高信号,囊变坏死区可表现为明显高信号,出血区呈低信号;2个病灶呈软组织肿块,信号尚均匀,T1WI呈等信号,T2WI呈稍高信号;增强扫描后呈实质部分明显强化。结论多形性未分化肉瘤的MR表现具有一定的特征性,MR对多形性未分化肉瘤有一定的诊断价值。Objective To investigated the MR appearance of undifferentiated pleomorphic sarcoma. Methods 12 lesions of eleven cases with pathologically confirmed UPS were analyzed retrospectively. All cases were performed MR examination. Results Of the 11 cases of pleomorphic undifferentiated sarcoma, 6 cases were located in thigh, 1 case in calf, 1 case in upper arm, 1 case in humeral back, 1 case in retroperitoneal. Four lesions presented with clear boundary, eight with unclear edge, all the leisions with the different degree of edema in the arround soft tissue. 10 lesions were inhomogeneous signal, eight lesions were shown low, iso-signals intensity on T1WI, high signal can be seen in one lesion, which prompt tumor with bleeding, isointensity or slightly high signal intensity on T~ WI fat suppression sequence, necrotic area can be characterized by high signal obviously, bleeding area with low signal. The even signals can be seen in 2 cases, slightly high signal on T2-weighted imaging. The sloid parts on the tumors had been obvious enhancement. Conclusion MR manifestation of pleomorphic undifferentiated sarcoma has certain characteristics. MR has a certain value in the diagnosis of undifferentiated pleomorphic sarcoma.
