涎腺黏膜相关淋巴组织结外边缘区淋巴瘤伴导管内生长3例临床病理观察

摘　　要：目的探讨涎腺黏膜相关淋巴组织结外边缘区(MALT)淋巴瘤伴导管内生长的临床病理学特点。方法对3例伴导管内生长的涎腺MALT淋巴瘤的组织病理学、免疫组化结果进行分析,并复习相关文献。结果 3例具有一般MALT淋巴瘤的病理学特征,包括中心细胞样细胞、单核样B细胞,滤泡植入及淋巴上皮病变。特殊之处在于肿瘤细胞在多个扩张的导管内生长,且充满整个导管,导管内肿瘤细胞形态单一,呈单核样B细胞外观,免疫组化导管内肿瘤细胞CD20及bcl-2(+)。结论涎腺MALT淋巴瘤伴肿瘤细胞导管内生长,这种特殊的形态学特征也许对于诊断MALT淋巴瘤有一定意义,但尚需收集更多的样本来验证。Objective To investigate the clinicopathological features in mucosa-associated lymphoid tissue （MALT） lymphoma of the salivary gland, with involvement of intraglandular ducts. Methods The histological findings and immunophenotype in 3 cases of MALT lymphoma with involvement of intraglandular ducts were evaluated with review of the relevant literatures. Results All 3 cases had commom histopathological features of MALT lymphoma including centrocyte-like cells, monocytoid B cells, and lymphoepithelial lesion. Monotonous monocytoid B cells grew within multi- dilated ducts were unusual. Immunophenotypically, the tumor cells within the ducts were positive for CD20 and bcl-2. Conclusions MALT lymphoma of the salivary gland with involvement of intraglandular ducts has a special pathological morphology which may be helpful in diagnosis and differential diagnosis of MALT lymphoma, but hypothesis should be tested by collecting more samples.
