改良式导弓治疗儿童乳前牙反牙合的头影测量分析

摘　　要：目的：探讨儿童乳前牙反[牙合]矫治的最佳时机及改良式导弓活动矫治器的治疗效果。方法选择3～5岁乳前牙反[牙合]患儿50例，采用改良式导弓活动矫治器进行正畸治疗，治疗前后拍头颅侧位片，通过头影测量分析，观察并比较患儿矫治前及矫治后上、下颌骨的位置变化（SNA、SNB、ANB、SN-Pg）、恒牙牙胚位置变化（U1/SN、U1/NA、I1-NB、I1/NB、U1-NA）及软硬组织变化。结果在经过14～75 d的矫治后，50例患儿均取得了良好的疗效。SNA增加（2．51±0．18）°，SNB减少（2．1±0．4）°，SNG增加（1．78±0．9）°，NLA减少（2．04±3．87）°，治疗后上下颌骨、恒切牙牙胚位置、软组织均得到不同程度改善。结论采用改良式导弓活动矫治器对乳前牙反[牙合]进行矫治，能够有效改善上下颌骨、恒切牙牙胚及软组织位置，并且疗程短，见效快，疗效满意。Objective To investigate the best timing of correcting primary dentition anterior crossbite and e-valuate the effects of modified interocclusal lip arch appliance in correcting primary dentition anterior crossbite.Methods Fifty cases （3-5 years old）with primary dentition anterior crossbite were selected. They were treated with the modified interocclusal lip arch appliance.Cephalometric radiographs were taken and analyzed at the pre-treatment time and the post-treatment time.We observed and compared the chan-ges of hard and soft tissue.Results After 14-75 days′correction,50 cases achieved good therapic results. SNA increased （2.51±0.18）°,SNB decreased by （2.1±0.4）°,SNG increased by （1.78±0.9）°,and NLA dreased by （2.04±3.87）°.All of the anterior crossbite were corrected and the profiles were improved satis-factorily after treatment.Conclusion The primary dentition anterior crossbite should be corrected in the milk teeth period.The best timing of correcting primary dentition anterior crossbite is 3-5 years old. Modified interocclusal lip arch appliance is a clinical effective method for correcting primary dentition ante-rior crossbite,improving anterposterior j aw relationship and the face profile.
