无痛胃镜下食管静脉曲张套扎术的安全性分析

摘　　要：【目的】比较常规胃镜与无痛胃镜下食管静脉曲张套扎术的安全性。【方法】研究纳入204例接受食管静脉曲张套扎术的肝硬化患者,其中无痛胃镜组135人,常规胃镜组69人;分别观察两组术前、术中及术后的心率、血压、血氧饱和度变化,以及术中不良反应以及完成手术所用的时间;同时评估手术者对术中内镜视野及操作的满意度。【结果】无痛胃镜组患者术中与术后的心率、血压、血氧饱和度分别与术前比较无统计学差异;同时分别与常规胃镜治疗组比较,术中心率、血压、血氧饱和度与术前比较也无统计学差异。无痛内镜组的术中不良反应(5.9%)显著低于常规内镜组(26.1%,P<0.01)。内镜操作时视野评估显示无痛胃镜组的满意度明显高于常规胃镜组(P<0.01)。无痛胃镜组的手术时间为(28.3±8.6)min,显著低于常规胃镜治疗组的(41.5±11.8)min(P<0.01)。【结论】无痛胃镜下食管静脉套扎术是安全的,不良反应低,术中视野清晰便于操作,易于患者接受。[Objective]To compare the safety of esophageal varices ligation （EVL） between the ordinary endoscopy and the sedation endoscopy.[Method]It was a retrospective study.A total of 204 patients with liver cirrhosis,who prefer to EVL,were enrolled and divided into the sedation endoscopy group （n =135）,the ordinary endoscopy group （n =69）.The heart rate,blood pressure and blood oxygen saturation were observed and recorded in pre-operation and intra-operation as well as post-operation.Adverse reactions and operative time were evaluated,and operator satisfaction of endoscopic view was evaluated after operation.[Results]In the sedation endoscopy group,the heart rate,blood pressure and blood oxygen saturation were not significantly different compared in pre-operation and intra-operation as well as post-operation,compared to the ordinary endoscopy group.Adverse reactions of the sedation endoscopy group （5.9%） were lower than the ordinary endoscopy group （26.1%,P 〈 0.01）.In the sedation endoscopy group,the satisfaction of endoscopic view was better than the ordinary endoscopy group,and the operative time also was less than the ordinary endoscopy group.[Conclusion]Esophageal varices ligation with sedation is safe and less adverse reactions,which might be more acceptable for both patients and the operators.
