应用十二指肠镜治疗胃内特殊部位息肉56例体会

摘　　要：目的:探讨应用十二指肠镜高频电治疗胃内特殊部位息肉的安全性、可行性及其疗效。方法:56例胃内特殊部位息肉普通胃镜(前视内镜)治疗困难,应用十二指肠镜(侧视内镜)行高频电治疗,术后送病理检查,记录并发症发生情况及疗效。结果:胃内特殊部位息肉56例,手术成功53例,成功率94.6%。其中贲门下41例(38/41),胃体小弯侧15例(15/15),息肉大小0.4-1.5cm,平均0.9cm,均一次切除。手术时间18-30min,平均20min,无出血及穿孔等严重并发症。术后病理:增生性息肉38例,炎性息肉6例,腺瘤性息肉2例,胃底腺息肉6例,组织学阴性10例。结论:十二指肠镜在胃内特殊部位息肉治疗方面有明显的优势,具有良好的安全性及疗效,值得同行借鉴。Objective： To explore the safety, feasibility and healing efficacy in using duodenoscope for high-frequency electric treatment of polyps in special location of gaster. Methods：56 cases of gastric polyps in special location,which were difficult to cure through forward-viewing gastroscopey, were operated by duodenoscopy Make pathologic examination, record incidence of complications and healing efficacy after operation. Results：The special location of the 56 gastric polyps. 53 ca- ses were sucessed, the rate is 94.6%. 41 was lower earida（ 38/41 ） , 15 was in lesser curvature of gaster （ 15/15 ）. the size of polyps was between O. 4 - 1.5cm. the mean size was 0.9cm. The operative time was between 18 - 30rain. mea,1 time was 20min. no serious compilications occurred,such a.s bleeding and perforation. Pathologic type： 38 was hyperplastic ployp. 6 was inflammatory polyp. 2 was adeuomatous polyp. 6 was fundic ghmd polyp. 10was histologically negative. Conclusion： Treating polyps in special location of gaster by duodenoseope has obvious advantage , it is safe anti effective. This method is worth learning for other Endoscopic Peers.
