一期结肠癌根治切除吻合口近端造口术处理左半结肠癌伴梗阻的临床应用

摘　　要：目的探讨一期结肠癌根治性切除吻合口近端造口术在治疗左半结肠癌伴梗阻患者中的临床应用效果。方法回顾75例左半结肠癌伴急性梗阻患者的临床处理过程,分析患者术后出现切口感染、吻合口瘘、肺部感染、盆腔感染发生情况及平均住院时间等。结果 75例患者术后发生切口感染3例,吻合口瘘1例,肺部感染2例,心功能衰竭1例,所有患者术后造瘘口还纳成功,平均住院12.5 d。结论左半结肠癌伴急性梗阻的患者行一期根治性切除吻合口近端造口术降低了单纯一期吻合术后吻合口瘘、切口感染等并发症的发生,改善了患者生活质量,是安全有效的治疗方式。Objective To investigate the clinical application effect of first stage colon cancer radical resection excision anastomosis ostomy in the treatment of left colon cancer with obstruction. Methods The clinical processes of 75 cases with left colon cancer with obstruction were summarized, and the postoperative wound infection, anastomotic fistula, pulmonary infection, pelvic infection and the average time in hospital were analyzed. Results Among the 75 cases, there were 3 cases with wound infection, 1 case with anastomotic fistula, 2 cases with pulmonary infection, and 1 case with heart failure. The ostomy succeeded for all the patients. The average time in hospital was 12.5 day. Conclusion First stage colon cancer radical resection excision anastomosis ostomy in the treatment of left colon cancer with obstruction can decrease the incidences of postoperative wound infection, anastomotic fistula, and other complications. It can also improve the life quality of patients, and is a safe and effective treatment.
