髋部锁定加压接骨板(LCP)治疗小儿髋关节发育异常

摘　　要：目的:探讨小儿髋部锁定加压接骨板(LCP)治疗小儿髋关节发育异常(DDH)的疗效优势。方法:回顾性分析在我院行LCP治疗的DDH患者20例(22髋),根据内固定系统的不同,分为角度钢板固定组(对照组)21髋和LCD固定组(观察组)22髋。比较两种固定方式对股骨截骨矫正前倾角以及颈干角后的固定疗效。结果:两组患儿术后前倾角及颈干角与术前比较有明显改善(P<0.05),但是观察组改善情况优于对照组(P<0.05)。术后根据Mckay标准对髋关节功能进行评分,对照组:优11髋(52.4%),良5髋(23.8%),可4髋(19.0%),差1髋(4.8%),优良率为76.2%;观察组:优15髋(68.2%),良5髋(22.7%),可2髋(9.1%),差0髋(0%),优良率为90.9%,两组患者优良率较差异有统计学意义(P<0.05)。结论:髋部锁定加压接骨板治疗小儿髋关节发育异常固定牢靠,患儿髋关节功能恢复满意,愈合良好,是一种安全可行的手术固定方法。Objective：To investigate the clinical advantage in the treatment of paediatric developmental dysplasia of hip using locking compression plate. Methods：20 patients（22 hips） being treated with LCP were retrospectively analyzed. The pa- tients were divided into the treatment group and the control group according to the difference of internal fixation system. The treatment group（21 hips） were treated with angle plate and the control group（22 hips） with locking compression plate. The fixation efficacy of the neck-shaft angle and the anteversion angle were being compared between two groups after opera- tion. Results：The neck-shaft and the anteversion angle were significantly different before and after treatment（P〈0.05）, and the improvement in treatment group was better than that in control group（P〈0.05）. According to Mckay score system, the control group were excellent in 11 cases（52.4%） ,good in 5 cases（23.8% ） ,fair in 4 cases（19.0%） and poor in 1 case （4.8%）, the excellent and good rate was 76.2%. The treatment group were excellent in 15 cases（68.2%）,good in 5 cases （22.7%）,fair in 2 eases（9.1%） and poor in 0 case（0%） ,the excellent and good rate was 90.9%. The difference was sig- nificant between the two groups（P〈0.05）. Conclusion：The treatment of the paediatric developmental dysplasia of hip using locking compression plate is a safe and perfect method for it＇s rigid fixation , satisfied recovery of hip function and wound healing.
