p27kip1蛋白在结肠癌组织中的表达及意义

摘　　要：目的探讨p27kip1蛋白在结肠癌组织中的表达及意义。方法应用免疫组化方法检测结肠癌组织(观察组40例)及癌旁正常组织(对照组20例)中p27kip1蛋白的表达,比较其阳性表达率,并比较分析观察组不同分化程度及有无淋巴结转移患者p27kip1蛋白的阳性表达程度。结果观察组中p27kip1蛋白阳性表达16例(40.00%),对照组中p27kip1蛋白阳性表达15例(75 00%),差异有统计学意义(P<0.05)。高、中分化患者p27kip1蛋白的阳性表达程度明显高于低分化患者,差异有统计学意义(P<0.05)。有淋巴结转移患者p27kip1蛋白的阳性表达程度明显低于无淋巴结转移患者,差异有统计学意义(P<0.05)。TNM分期Ⅰ~Ⅱ期患者p27kip1蛋白阳性表达12例(63.16%),Ⅲ~Ⅳ期患者p27kip1蛋白阳性表达4例(19.05%),差异有统计学意义(P<0.05)。结论 p27kip1蛋白作为抑癌因子具有调控细胞增殖、分化及凋亡的能力,而组织中p27kip1蛋白的低表达可能导致了肿瘤的发生、发展。p27kip1蛋白可能作为临床上检测结肠癌的新指标,有助于结肠癌的肿瘤分化、肿瘤转移及肿瘤TNM分期的诊断。Objective To study the express of p27kip1 in carcinoma of colon,then to investigate its roles and probable mechanism in the pathogenesis of colon cancer. Methods Immunohistochemistry was used to detect the expression and distri-bution of p27kip1 in carcinoma of colon and normal colon. Statistics was used to analyze the data.Making a analysis of positive expression level of p27kip1 protein in colon caner tissue with different degree of differentiation and lymph node metastasis. Re-sults 16 cases of positive expression of p27kip1 Protein in observation group （40%）, 15 cases of positive expression of p27kip1 Protein in the control group （75%）, the difference was statistical y significant （P〈0.05）.The expression of p27kip1 Protein in group of high degree of differentiation was significantly higher than that group of low differentiated. The difference was statistical y sig-nificant （P〈0.05）. The positive expression of p27kip1 Protein in patients with the extent of lymph node metastasis was significantly lower than that in patients without lymph node metastasis, the difference was statistical y significant （P〈0.05）. 12 cases of positive expression of p27kip1 Protein in TNM stage I^II （63.16%）, 4 cases of positive expression of p27kip1 Protein in TNM stageⅢ~Ⅳ（19.05%）, the difference was statistical y significant （P〈0.05）. Conclusion p27kip1 protein as a tumor suppressor factor that can regulate cellproliferation, differentiation and apoptosis of ability. Low expression of p27kip1 protein led to tumor occurrence and development in organization.Protein of p27kip1 would be considered as new clinical index of colon carcinoma, and make for di-agnose of neoplasm differentiation, neoplasm metastasis and TNM staging.
