β1整合素在结直肠癌肝转移组织中的表达及其临床意义

摘　　要：目的 观察结直肠癌肝转移中β1整合素表达与临床病理特征的关系,探讨β1整合素在结直肠癌肝转移中的意义.方法 收集41例未行新辅助化疗的结直肠癌肝转移手术切除标本及其临床病理资料.采用苏木素-伊红（HE）染色检测结直肠癌肝转移标本中β1整合素的表达,分析结直肠癌肝转移中β1整合素表达与临床病理特征的关系.结果 41例中有29例（70.7％）肝转移癌β1整合素表达阳性.29例中28例（96.6％）有脉管侵犯,明显多于无脉管侵犯者（1例,3.4％）;26例（89.7％）有淋巴结转移,明显多于无淋巴转移者（3例,10.3％）;Ⅲ期（9例）＋Ⅳ期（16例）共25例（86.2％）,明显多于Ⅰ期（0例）＋Ⅱ期（4例）共4例（13.8％）;中分化18例（62.1％）＋低分化11例（37.9％）,明显多于高分化者（0例）,差异均有统计学意义（P＜0.05）.结论 结直肠癌肝转移中β1整合素表达与原发癌临床病理特征有相关,β1整合素在结直肠癌肝转移的发生发展中起重要作用.Objective To discuss the significance of β1 integrin expression in hepatic metastasis of colorectal cancer by observing the correlation between β1 integrin expression in colorectal liver metastasis and clinicopathological characteristics.Methods Forty-one hepatectomy specimens of liver metastasis from colorectal cancer without neoadjuvant therapy were chosen for detection of the expression of β1 integrin by the hematoxylin and eosin （HE） immunohistochemical techniques.The clinicopathological characteristics of 41 cases were analyzed.Results There were 29 specimens （70.7％） positive for expression of β1 integrin in 41 cases.In the 29 cases,there were 28 （96.6％） cases of vascular invasion in primary colorectal cancer,26 （89.7％） case of lymphatic metastasis,25 （86.2％） cases of later TNM stages （9 cases of stage Ⅲ,and 16 cases of stage Ⅳ）,18 （62.1％） cases of moderate differentiation and 11 （37.9％） cases of poor differentiation.There were statistically significant differences （P ＜ 0.05）.Conclusion The expression of β1 integrin in colorectal liver metastasis may be closely associated with the clinicopathological features,suggesting that β1 integrin may play a role in hepatic metastasis from colorectal cancer.
