摘  要:目的:了解产前系统超声检查对先天畸形的筛查效果。方法2010年1月-2012年12月在产科门诊接受胎儿产前B超筛查,并在该院住院分娩或终止妊娠的2056例孕妇的筛查结果,通过尸检、临床特征及染色体检查等确诊结果资料为基础,计算产前系统超声筛查先天畸形的灵敏度、特异度、阳性预测值和阴性预测值等。结果2056例孕妇参加了产前筛查,共分娩或引产胎儿2056例,产前正确筛查出75例,确诊为先天畸形患儿86例。产前系统超声筛查对先天畸形诊断的灵敏度、特异度分别为87.2%,99.8%,阳性预测值、阴性预测值分别为93.8%,99.4%,与产后临床诊断的符合率为99.2%;对严重结构畸形的诊断灵敏度、特异度分别为94.1%,99.9%,阳性预测值、阴性预测值分别为98.0%,99.9%,与产后临床诊断的符合率为99.9%。结论产前系统超声检查对先天畸形和严重结构畸形的筛查具有较高的临床价值,为临床处理提供早期的有利依据,减少严重畸形新生儿的出生率。Objective To assess the value of prenatal ultrasound system screening for congenital malformations. Methods Data of prenatal screening results for 2 056 women with singleton pregnancy in our hospital from January 2010 through December 2012 were analyzed.Fetal outcomes were ascertained through autopsy,physical examination and chromosome test.Sensitivity,specificity,positive predictive value and negative predictive value of prenatal ultrasound screening system for congenital malformations were calculated. Results Of the 2 056 fetuses,86 fetuses were diagnosed as having a defect.Of the 86 fetuses,prenatal ultrasound correctly detected 75 cases,resulting in sensitivity of 87.2%and specificity of 99.8%,positive predictive value of 93.8%,negative predictive value of 99.4%,as well as the clinical diagnosis consistent rate of 99.2%.For major structural deformities,the sensitivity was 94.1%,specificity 99.9%, positive predictive value 98.0%,negative predictive value 99.9%,as well as the clinical diagnosis consistent rate 99.9%. Conclusion Prenatal ultrasound system screening for congenital malformations is valuable in detecting major fetal defects.
