肿瘤标志物检测在肺癌诊断中的临床意义

摘　　要：目的:评价癌胚抗原(CEA)、血清细胞角蛋白19片段抗原(CYFRA21-1)、糖类抗原125(CA125)及神经元特异烯醇化酶(NSE)水平在肺癌临床分型、判断病理类型中的意义。方法:对肺癌组104例、肺良性病变组50例患者的CEA、CYFRA21-1、NSE、CA125进行综合比较。结果:肺癌组患者CEA、CYFRA21-1、CA125、NSE水平均高于肺良性病变组,差异有统计学意义(P<0.05);CEA在腺癌IV期中阳性率最高,CYFRA21-1在腺癌Ⅳ期、鳞癌Ⅳ期中阳性率最高,CA125在腺癌Ⅰ～Ⅳ期阳性率均最高,且腺、磷癌中CEA、CYFRA21-1临床分期越晚,阳性率越高,存在明显差异。结论:CEA、CYFRA21-1、CA125、NSE均可作为肺癌筛查的指标。另外,不同肿瘤标志物的阳性率对临床分型、病理类型有一定的参考意义。Objective：To investigate the significance of the quantity of the serum CEA,CYFRA21-1,CA125 and NSE of patients with lung cancer in the clinical types and pathological types of lung cancer.Method：The serum samples of 104 patients with lung cancer and 50 patients with benign lung diseases were measured.Result：The quantity of the serum CEA,CYFRA21-1,CA125 and NSE of patients with lung cancer were higher than that of patients with benign lung diseases.The positive rate of CEA in stageⅣadenocarcinoma was highest.The positive rate of CYFR21-1 in stageⅣadenocarcinoma and stageⅣsquamous cell carcinoma was higher than other stages.The positive rate of CA125 in stageⅠ-Ⅳadenocarcinoma was higher than other lung cancers.The positive rate of CEA and CA125 and the clinical stage of adenocarcinoma and squamous cell carcinoma were significantly correlated.Conclusion：CEA,CYFRA21-1,CA125 and NSE can be used as screening indicators of lung cancer.The level of tumor markers was significantly correlated with the clinical stage and pathological type of lung cancer.
