甲状腺微小癌健康筛查的超声征象分析

摘　　要：目的：探讨甲状腺微小癌健康筛查的超声征象。方法回顾性分析86个甲状腺微小癌与84个≤1cm的甲状腺良性结节的灰阶超声表现，比较分析两组间结节的回声、形态、边界、纵横比、内部成分、声晕、微小钙化，比较各组间超声征象的敏感性、特异性、准确性。结果两组间极低回声、形态不规整、实性回声、纵横比≥1、不规则声晕、微小钙化差异有统计学意义。使用极低回声、纵横比≥1、微小钙化这三项特异性高的超声征象作为联合诊断指标，敏感性为88.4%，特异性为76.2%，准确性为82.4%。结论极低回声、纵横比≥1、微小钙化三项超声指标联合运用，在健康体检中能快速、有效地筛查出甲状腺微小癌。Objective To explorer the ultrasonic signs of health screening in the diagonosis of thyroid microcarcinoma （TMC）.MethodsThe sonographic characeters of 86 TMC and 84 benign thyriod nodes（≤1cm）were retrospectively reviewed and compared with each other,incluededing echogenicity,shape,boundary,anteroposterior to transverse dimension ratio（A/T）,internal component,halo sign,microcalcifi cation and so on.Sensitivity,specifi city,accuracy among the ultrasonic signs were compared.Results There were signifi cantly statistical differences between the malignant and benign nodules,which were marked hypoecho,irregular sharpe,solid lesion,A/T≥1,irregular halo sign and microcalcifi cation.When using marked internal hypoechoic echo,A/T≥1 and microcalcificaion as a new combined criterior,the sensitivity,spicificity and accuracy were 88.4%,76.2% and 82.4% respectively.Conclusions The combined criterior using marked hypoechoic echo,A/T≥1 and microcalcifi caion can rapidly,effectively screening the TMC in health examination.
