摘 要:目的:检测宫颈液基细胞学(LCT)检查剩余标本组织块中P16蛋白和胰岛素样生长因子ⅡmRNA结合蛋白3(IMP3)蛋白的表达水平,探讨其用于宫颈病变诊断的病理学意义。方法:采用免疫组化法检测87例患者宫颈脱落细胞块中P16蛋白和IMP3蛋白的表达情况。结果:P16蛋白在LCT细胞块中的阳性率分别为无上皮内病变(NILM)0.00%、非典型鳞状上皮细胞(ASCUS)31.03%、低度鳞状上皮内病变(LISL)54.17%、高度鳞状上皮内病变(HISL)95.00%和鳞状细胞癌(SCC)100.00%;IMP3蛋白在LCT细胞块中的阳性率分别为NILM 0.00%、ASCUS 24.14%、LISL 50.00%、HISL 85.00%和SCC 85.71%,P16蛋白和IMP3蛋白在HSIL组和SCC组的阳性表达率均高于其他组,差异有统计学意义(P<0.05);P16蛋白与IMP3蛋白的表达与宫颈病变程度呈正相关,随病变级别的增高而增高;HISL中P16蛋白与IMP3蛋白表达呈正相关。结论:P16蛋白和IMP3蛋白在宫颈病变的细胞学诊断中具有重要意义,二者联合检测能提高LCT的检出率,有效分流不同宫颈病变级别的患者,指导治疗和监测。Objective: To detect the expression levels of P16 protein and insulin - like growth factor II mRNA binding protein 3 ( IMP3 ) in remaining cervical cell block after LCT, explore the pathological significance for diagnosis of cervical lesions. Methods: Immu- nohistoehemical method was used to detect the expression levels of P16 protein and IMP3 protein in remaining cervical cell block in 87 patients. Results: The positive expression rates of P16 protein in cervical cell block in NILM, ASCUS, LISL, HISL and SCC were 0. 00%, 31.03% , 54. 17% , 95.00% and 100.00% , respectively. The positive expression rates of IMP3 protein in cervical cell block in NILM, ASCUS, LISL, HISL and SCC were 0. 00%, 24. 14%, 50.00% , 85.00% and 85.71% , respectively. The positive expression rates of P16 protein and IMP3 protein in cervical cell block in HSIL and SCC were statistically significantly higher than those in the other groups ( P 〈 0.05 ) ; there was a positive correlation between expressions of P16 protein and IMP3 protein and degree of cervical lesions, the expressions of P16 protein and IMP3 protein increased with the increase of degree of cervical lesions; in patients with HISL, there was a positive correlation between P16 protein expression and IMP3 protein expression. Conclusion: P16 protein and IMP3 protein have important significance in cytological diagnosis of cervical lesions, joint detection of P16 protein and IMP3 protein can improve the detection rate, effectively grade patients with different cervical lesions and direct treatment and monitoring.
