摘  要:目的:开发骨龄智能评测系统,通过其临床应用,评价该系统的准确性和实用性。方法:选取100例6—13岁儿童为研究对象,利用计算机图形学和半拉格朗日算法对骨龄片的目标兴趣区(ROI)进行图像分析,同时有2名小儿内分泌专家(A、B)采用TW3法人工判读骨龄(两种方法均检测2次)。结果:A、B不同判读者间评估骨龄的不确定度有显著性差异(P〈0.05),A、B专家间RUS(包括尺挠骨及掌指骨)骨龄有显著性差异(P〈0.05),而腕骨骨龄无明显差异(P〉0.05);骨龄智能评测系统的检测结果不确定度(0.12岁)显著低于人工判读的A、B专家(P〈0.05),RUS骨龄与B专家人工判读有显著性差异(P〈0.05),腕骨骨龄无明显差异(P〉0.05);与A专家的RUS骨龄、腕骨骨龄均无显著差异(P〉0.05)。结论骨龄智能评测系统克服了传统人工判读骨龄的人为误差,降低了骨龄评估结果的不确定度,提高了骨龄评估的速度和准确性,值得临床推广。Objective: To develop hone age intelligent assessment system, evaluate the accuracy and practicability of the system through its clinical application. Methods: A total of 100 children aged 6 - 13 years old were selected as study objects, computer graphic method and semi -Lagrangian integration scheme were used to conduct image analysis on regions of interest (ROI) ; TW3 method was used to determine bone age by two pediatric endocrinologists ( A and B), the examinations were conducted twice. Results: There was statistically significant difference in uncertainty of bone age assessment results between pediatric endocrinologist A and pediatric endocrinologist B (P 〈 0.05) ; there was statistically significant difference in RUS bone age ( including ulna and radius, metacarpale and phalanges) reported by pediatric endocrinologists between A and B (P 〈 0. 05 ), but there was no statistically significant difference in bone age of carpal ( P 〉 0. 05 ) ; the uncertainty of bone age intelligent assessment system (0. 12 year) was statistically significantly lower than that of artificial reading ( pediatric endocrinologist A and B) (P 〈 0. 05) ; there was statistically significant difference in RUS bone age between bone age intelligent assessment system and artificial reading (pediatric endocrinologist B) (P 〈 0. 05 ), but there was no statistically significant difference in bone age of carpal ( P 〉 0. 05 ) ; there was no statistically significant difference in RUS bone age and bone age of carpal between bone age intelligent assessment system and artificial reading (pediatric endocrinologist A) (P 〉 0. 05) . Conclusion: Bone age intelligent evaluation system overcomes the personal error of traditional manual interpretation, reduces the uncertainty of bone age assessment results, improves the speed and accuracy of bone age assessment, which is worth clinical popularization.
