GP73、AFP-L3和AFP联合检测在原发性肝癌诊断中的价值

摘　　要：[目的]探讨高尔基体蛋白73（golgi protein 73,GP73）、甲胎蛋白异质体L3 （alpha-fetoprotein-L3,AFP-L3）和甲胎蛋白（alpha-fetoprotein,AFP）联合检测在原发性肝癌（PHC）诊断中的应用.[方法]选取2012年2月至2013年10月在本院就诊患者112例,其中PHC组55例,肝炎组25例,肝硬化组32例,另选择肝脏功能正常者20例作为对照组.用酶联免疫法检测患者血清中GP73浓度,化学发光法检测AFP和AFP-L3的浓度,并计算AFP-L3在AFP中所占比例.[结果]GP73、AFP-L3与AFP在对照组、肝硬化组、肝炎组的表达水平均低于PHC组,且差异有统计学意义（P＜0.01）;使用GP73＋ AFP与AFP-L3＋ AFP对PHC诊断的灵敏度高于单独使用AFP（92.7％ vs 70.9％,P＜0.01;89.1％ vs 70.9％,P＜0.05）,而特异性基本无变化.[结论]联合检测GP73、AFP-L3和AFP,能提高对PHC的早期诊断率,对临床工作有重要指导意义.[Objective]To explore the application of the combined detection of golgi protein 73（GP73）,alph-fetoprotein-L3（AFP-L3） and alpha-fetoprotein（AFP） in the diagnosis of primary hepatic carcinoma（PHC）.[Methods] Totally 132 patients in our hospital from Feb.2012 to Oct.2013 were chosen and divided into PHC group（n =55）,hepatitis group（n =25）,liver cirrhosis group（n =32） and normal hepatic function group（n =20）.Enzyme-linked immunosorbent assay（ELLSA） was used to detect the level of GP73.Chemiluminescence method was used to examine the levels of AFP-L3 and AFP,and then the proportion of AFP-L3 in AFP was calculated.[Results] The levels of GP73,AFP-L3％ and AFP in healthy group,liver cirrhosis group and hepatitis group were lower than those in PHC group,and there were significant differences（P ＜ 0.01）.The sensitivity of GP73＋ AFP and AFP-L3＋ AFP for the detection of PHC were higher than that of AFP alone（92.7％ vs.70.9％,P ＝0.006; 89.1％ vs.70.9％,P=0.032）,but the specificity had no difference basically.[Conclusion] The combined detection of GP73,AFP-L3 and AFP can improve the early diagnostic rate of PHC,and is of significance in guiding clinical practice.Therefore,it has wide application prospect.
