摘 要:目的将个性化治疗技术应用于颌面部骨折修复,提高手术的精确度和安全性并减少手术时间。方法收集颌面部骨折患者40例,随机分为A、B两组,各20例。A组应用个性化治疗技术治疗颌面部骨折修复;B组经重建后得到三维立体模型,按照以往经验,于术中根据三维重建的图像行常规固定颌面部骨折。对比A、B两组术前、术后三维CT重建的图像,从冠状面、矢状面、横断面上分别测量及评估骨折固定效果。结果所有患者均恢复原有咬合关系,A组患者骨折复位与术前设计基本一致,术后颌骨平均位移为(1.53±0.17)mm,其中固定钛板时间为每板钉(36.20±0.45)s;B组患者术后大部分保证了颌骨的解剖复位,术后颌骨平均位移为(3.53±0.57)mm,其中固定钛板时间为每板钉(60.50±0.75)s。结论个性化治疗技术可有效地提高颌面部骨折手术的精确度和安全性,并减少手术时间。Objective The application of personalized therapy of maxillofacial fracture repair, may improve the accuracy and safety of operation and reduces surgery time. Methods 40 patients with maxillofacial fractures were collected, and were randomly divided in tb A, B two groups. Patients in group A were treated with personalized therapy ; while three-dimensional model was obtained after reconstruction of group B patients, and maxillofacial fractures were operated according to past experience in operation. Then 3D CT reconstructions were performed in every patient. The assessments of fracture fixation were gained from the coronal, sagittal, transverse sections. Results All the patients were restored to their original occlusal rela- tionship, and the fracture reduction patients in group A were consistent with basic preoperative design, the average postoper- ative jaw displacement was ( 1.53± 0.17 ) mm, the fixing-plate time was ( 36.20 ± 0.45 ) s/plate / screw ; anatomical re- duction were almost done in group B, the average postoperative jaw displacement was (3.53± 0.57)mm, the fixing-plate time was (60.50±0.75 ) s/ plate / screw. Conclusion Personalized therapy can effectively improve the precision and safety of operation for maxillofacial fracture and reduce operation time.
