摘 要:目的:探讨消化系肿瘤多耐药基因1(multidrug resistance gene 1,MDR1)、P-糖蛋白(P-glyco-protein,P-gp)的表达与化疗药物耐药的相关关系.方法:本次研究选取120例消化系肿瘤患者的手术标本,并且采取荧光定量的方法检测MDR1基因表达水平,而采取ELISA法检测P-gp表达水平,并选取同期120例非肿瘤患者为对照组.并将其与ATP生物荧光肿瘤体外药敏检测技术(ATP-TCA)化疗药物的敏感性结果进行比较.结果:研究组肿瘤MDR1 mRNA表达阳性率为77.5%(93/120),对照组肿瘤MDR1 mRNA表达阳性率为25.8%(31/120),研究组肿瘤M D R1 m R N A表达阳性率明显的高于对照组,差异有统计学意义(P<0.05).研究组P-gp表达阳性率为65.0%(78/120),对照组P-gp表达阳性率为20.0%(24/120),研究组P-gp表达阳性率明显的高于对照组,差异有统计学意义(P<0.05).10种化疗药物均不敏感14份,对1-3种化疗药物敏感36份,对4-9种化疗药物敏感28份,对10种化疗药物敏感2份.消化系肿瘤MDR1基因、P-gp表达率与化疗药物敏感度呈现负相关性,差异有统计学意义(P<0.05).结论:临床中肿瘤MDR1 mRNA、P-gp的表达常常与肿瘤耐药性具有相关性,且肿瘤MDR1 mRNA、P-gp的阳性表达提示患者很有可能对化疗药物产生耐药性,临床中应引起足够的重视.AIM: To assess the correlation between expres-sion of multidrug resistance gene 1(MDR1)gene and P-glycoprotein(P-gp) and resistanceto chemotherapeutic drugs in gastrointestinaltumors.METHODS: One hundred and twenty patientswith digestive tract tumors were included.Sur-gical specimens from these patients were usedfor fluorescence quantitative detection of MDR1 gene expression level.P-gp expression level wasdetected by ELISA.One hundred and twentynon-tumor patients treated during the same period were used as controls.ATP biolumines-cence in vitro susceptibility testing technologywas used to assess sensitivity to chemotherapeu-tic drugs.RESULTS: The positive rate of MDR1 mRNA expression was significantly higher in the study group than in the control group(77.5% vs 25.8%,P〈0.05).The positive rate of P-gly-coprotein expression was also significantly higher in the study group than in the control group(65.0% vs 20.0%,P〈0.05).There were 14 cases that were not sensitive to all the 10 chemotherapeutic drugs,36 cases that were sensitive to 1-3 chemotherapeutic drugs,28 cases that were sensitive to 4-9 chemothera-peutic drugs,and two cases that were sensitive to all the 10 kinds of chemotherapeutic drugs.There was a significant correlation between MDR1 gene and P-glycoprotein expression and sensitivity to chemotherapeutic drugs(P〈0.05).CONCLUSION: MDR1 mRNA and P-gp ex-pression correlates with tumor drug resis-tance,suggesting that patients with positive expression of P-gp are more likely to develop resistance to chemotherapeutic drugs.
