摘　　要：目的:探究肺癌根治术对患者血小板数量(PLT)、血小板比积(PCT)、血小板分布宽度(PDW)的变化。方法:选取本院胸外科诊断为原发性肺癌336例,术前及术后7d抽取静脉血分别检测PLT、PCT及PDW,分析肺癌根治术对血小板参数的影响。结果:术前肺癌PLT、PCT较正常人高,PDW较健康正常人低,差异有统计学意义(P<0.05);术后7dPLT、PCT较术前明显低下,差异有统计学意义(P<0.05);肺癌ⅠB期术前PLT、PCT、PDW较ⅠA期术前有明显差异,差异有统计学意义(P<0.05);肺癌ⅠB期术后7dPLT、PCT较ⅠA期术后7d高,差异有统计学意义(P<0.05);不同病理类型中小细胞癌术后7dPLT、PCT较其他类型高,差异有统计学意义(P<0.05)。结论:肺癌细胞可能产生某种类似血小板生成激素刺激骨髓生成血小板;肺癌根治术能够明显降低患者PLT,可能跟组织、血管破坏,短期内PLT耗损或者癌组织被清除有关;PLT、PCT、PDW的动态观察,有助于临床对肺癌病情和预后判断。Objective： To explore changes in blood PLT, PCT, PDW of lung cancer patients with radical treatment. Methods： A total of 336 primary lung cancer cases were selected. Venous blood platelet number （PLT）, platelet thrombocyt- oerit（PCT） and platelet distribution width （PDW） were detected before surgery and 7 days after surgery, and the effect of radi cal surgery on platelet parameters was also analyzed. Results：Before surgery, PLT and PCT were significantly increased, PDW was significantly decreased compared with normal persons （P〈0.05）. Seven days after surgery, PLT and PCT were significantly decreased （P〈 0.05）. PLT and PCT of patients at I B stage were significantly higher than those of patients of I A 7 days after surgery（P〈0. 05）. And PLT and PCT of patients with small cell carcinoma were significantly higher than those of the others（P〈0.05）. Conclusion.. Lung cancer cells may produce some hormones similar to thrombopoiesis hormones, which can stimulate bone marrow platelet production. Radical surgery can significantly decrease PLT. It may be associated with damage to tissue and blood vessels, PLT loss in the short term or removal of cancerous tissues. Dynamic monitoring of PLT, PCT and PDW can help assessment of prognosis.
