摘  要:目的:探讨联合测定血清乳酸脱氢酶(LDH)和血小板计数(PLT)对急性白血病早期诊断的临床价值,以提高急性白血病初诊的准确性和特异性。方法对初诊的急性白血病患者同步测定LDH、PLT,通过受试者工作特征(ROC)曲线分析其临床性能,确立两者联合测定对急性白血病的诊断临界值。结果 LDH、PLT以及联合应用LDH与PLT的曲线下面积分别为0.908,0.811,其可信区间分别为0.852-0.965,0.728-0.893,0.877-0.969。结论运用ROC曲线分析简单有效,血浆LDH和全血PLT的检测对急性白血病早期诊断有较好的临床价值,以LDH〉322 U/L,且PLT〈72×109/L为诊断临界值较为适合。Objective To study the clinical value of the joint determination of lactate dehydrogenases(LDH) and platelet count(PLT) in the early diagnosis of acute leukemia, in order to improve the accuracy and specificity of the early diagnosis of acute leukemia. Methods To do synchronous determination of LDH and PLT for the patients in Jiangmen Central Hospital, then to analyse the clinical performance of LDH and PLT joint determination by ROC curve , and establish its critical value in the diagnosis of acute leukemia. Results The area under the curve of LDH, PLT and LDH together with PLT were 0.908, 0.811 and 0.923. And their credible interval were 0.852-0.965, 0.728-0.893, 0.877-0.969. Conclusion ROC curve analysis is simple and effective, and the determination of plasma LDH and whole blood PLT has high clinical value in the early diagnosis of acute leukemia, and taking LDH〉322 U/L, and PLT〈72×109/L as the critical value of diagnosis is appropriate.
