TRPM8在胰腺导管腺癌中的表达及意义

摘　　要：目的检测TRPM8在胰腺导管腺癌中的表达情况并分析其临床意义。方法收集我院肝胆外科自2008年至2010年40例行手术切除的胰腺导管腺癌病例的临床病理资料,分别采用实时荧光定量PCR(Real-Time PCR)、免疫组织化学和Western blotting的方法检测TRPM8 mRNA和蛋白水平的表达情况,并分析其与各临床病理参数之间的关系。结果 TRPM8在mRNA和蛋白水平表达一致。TRPM8mRNA在胰腺导管腺癌患者癌组织中表达明显高于正常组织[(0.0512±0.0498)vs(0.0008±0.0005),P<0.05]。单因素和多因素分析发现TRPM8的表达是一个独立的预后因素。2年无病生存率和总生存率,TRPM8阳性染色组明显低于阴性染色组(48%vs 18%,58%vs 22%),差异具有统计学意义(P<0.05)。临床病理特征评估结果:TRPM8的高表达与胰腺导管腺癌组织的分化程度、TNM分期及淋巴结转移之间存在显著性相关(P<0.05),与肿瘤大小、患者年龄、性别等无关。结论 TRPM8在胰腺导管腺癌中表达上调并与一些临床病理特征密切相关,提示TRPM8可能是反映胰腺导管腺癌发生发展、侵袭、转移及预后的重要生物学指标。Objective To examine the expressions of transient receptor potential channel TRPM8 and its clinical significance in human pancreatic ductal adenocarcinoma （PDAC）.Methods The expression levels of IRPM8 on both steady-state mRNA and protein levels in 40 cases of PDAC samples were determined by quantitative real-time PCR and immunohistochemistry,respectively.The correlations between TRPM8 expression and various clinicopathological parameters were analyzed.Results The expression of TRPM8 on the mRNA level was consistent with that on the protein level.TRPM8 mRNA levels in tumor tissues were significantly higher than that in the paratumor tissues [（0.0512 ± 0.0498） vs （0.0008 ± 0.0005）; P＜0.05].Univariate and multivariate analyses revealed that TRPM8 expression was an independent prognostic factor for disease-free survival and overall survival.Both 2-year disease-free survival rates and overall survival rates in TRPM8-positive group were significantly lower than that in negative group （48％ vs 18％,58％ vs 22％）.Clinicopathological characteristics evaluation results showed that high mRNA expression of TRPM8 significantly correlated with the differentiation,TNM stage and lymph node metastasis of the tumor （P＜0.05）.No significant correlation was found between TRPM8 mRNA expression and other variables,including tumor diameter,patient age and gender （P＞ 0.05）.Conclusion Our results suggest that TRPM8 expression is up-regulated in PDAC and its expression is closely associated with several clinical and pathological features for PDAC.TRPM8 may be an important biological indicators and reflect the development,invasion,metastasis and prognosis of pancreatic ductal adenocarcinoma.
