聚桂醇泡沫硬化剂治疗儿童头颈部大囊型淋巴管畸形疗效研究

摘　　要：目的探讨聚桂醇泡沫硬化剂治疗儿童头颈部大囊型淋巴管畸形的疗效。方法对2010年1月至2012年12月收治的46例大囊型淋巴管畸形进行瘤内注射聚桂醇泡沫硬化剂治疗。1%聚桂醇原液通过涡流法按液气比例为1∶4制成泡沫硬化剂。抽取部分囊液后,根据淋巴管畸形大小调整用量,一般总量≤8 ml(25 mg)。分期治疗者1次/周,2次为一个疗程。2个疗程后间隔1个月观察疗效再进行下一个疗程。结果经过1个至多个疗程后,46例头颈部大囊型淋巴管畸形全部治愈。其中6例注射1次肿块完全消退,18例注射2个疗程,13例注射3个疗程,7例注射4个疗程。另2例注射4个疗程后2个月复发,行再次硬化治疗1个疗程,肿块完全消失。本组无严重并发症及不良反应发生。结论聚桂醇泡沫硬化剂局部硬化治疗儿童头颈部大囊型淋巴管畸形疗效高、无并发症及严重不良反应,最大程度保持了患儿容貌,是一种简便、安全、有效的非手术疗法。Objective To evaluate the efficacy of lauromacrogol foam sclerotherapy in treating large cystic lymphatic malformations at cephalocervieal region in pediatric patients. Methods During the period from Jan. 2010 to Dee. 2012, a total of 46 pediatric patients with large cystic lymphatic malformations at cephaloeervical region were admitted to authors＇ hospital. Lauromaerogol foam sclerotherapy was employed in all patients. By using Tessari method, 1% lauromacrogol and air （1：4 ratio） were mixed to prepare the foam hardening agent. After a part of cystic fluid was drawn out, the foam hard- ening agent was injected into the lesion, and the total dose of the foam hardening agent used in one procedure was≤8 ml （25 mg）. For patients receiving staged treatment the injection was performed once per week, and one course of treatment consisted of two times. After 2 courses of treatment, the patients were kept under observation for one month and the thera- peutic course would be taken once more if necessary. Results After one or more courses of treatment, the large cystic lymphatic malformations at cephaloeervical region was completely cured in all 46 patients. Among the 46 patients, only one course of treatment was performed in 6, two courses of treatment in 18, 3 courses of treatment in 13 and 4 courses of treat- ment in 7. Recurrence was seen in 2 patients, who had received 4 courses of treatment, and lauromacrogol foam selerother- apy had to be carried out once more, and the lesion was totally disappeared. No severe complications or side-effects oc- curred. Conclusion For the treatment of large cystic lymphatic malformations at cephalocervical region in pediatric pa- tients, lauromaerogol foam sclerotherapy, as a non-surgical management, is safe, simple and very effective with no severe complications or side-effects. The patient＇ s appearance can be preserved to the greatest possible advantage.
