摘 要:目的 探讨急性脑梗死患者基质金属蛋白酶-9(MMP-9)和胱抑素C(Cys-C)的变化及临床应用价值.方法 选取2011年9月~ 2013年9月我院收治的84例脑梗死患者作为研究对象,其中大梗死灶组(梗死体积>10 cm3)26例,中梗死灶组(4cm3≤梗死体积≤10 cm3)28例,小梗死灶组(梗死体积<4 cm3)30例;参照神经功能缺损程度评分分组:重型组(31 ~45分)23例,中型组(16~30分)28例,轻型组(0~15分)33例.另选取30例同期健康体检者作为对照组,比较脑梗死患者与对照组Cys-C、MMP-9水平.结果 相比于对照组,大梗死灶组及中梗死灶组Cys-C及MMP-9水平明显升高(P<0.05);大梗死灶组Cys-C及MMP-9水平明显高于中梗死灶组及小梗死灶组(P<0.05);小梗死灶组Cys-C及MMP-9水平与对照组比较无统计学差异(P>0.05).相比于轻型组及中型组,重型组Cys-C及MMP-9水平明显升高(P<0.05);中型组Cys-C及MMP-9水平明显高于轻型组(P<0.05).结论 Cys-C及MMP-9水平升高与脑梗死发生发展密切相关,检测两者水平有助于判断脑梗死病情.Objective To explore the changes of matrix metalloproteinase-9 (MMP-9) and cystatin-C (Cys-C) in patients with acute cerebral infarction and the value of clinical application.Methods 84 hospitalized cases with acute cerebral infarction were selected from Sept.,2011 to Sept.,2013,including 26 cases of large infarction focus with the volume > 10 cm3,28 cases of middle infarction focus with the volume from≥4 cm3 to≤10 cm3 and 30 cases of small infarction focus with the volume <4 cm 3; according to the degree of neurological deficit scores,the 84 cases were divided into 3 groups:severe group(from 31 to 45 points) (n =23),medium group (from 16 to 30 points) (n =28) and mild group (from 0 to 15 points) (n =33) ; 30 normal healthy were selected to be the control; the levels of Cys-C and MMP-9 of the cases with cerebral infarction and the control were compared.Results The levels of Cys-C and MMP-9 in cases of large and middle infarction focus increased much more than those in controllers (P < 0.05) ;the level of Cys-C and MMP-9 in cases of large infarction focus was obviously higher than those in cases of middle and small infarction focus(P <0.05) ;the level of Cys-C and MMP-9 in cases of small infarction focus was of no difference from that in controllers(P > 0.05) ;the level of Cys-C and MMP-9 in severe group increased much more than those in medium and mild group(P <0.05),while the level in medium group was much higher than that in mild group (P < 0.05).Conclusions There exists a close correlation between the increasing levels of Cys-C and MMP-9 and the occurrence and progress of cerebral infarction ; the detection of the levels of Cys-C and MMP-9 is of help in evaluating the state of cerebral infarction.
