腹腔镜下经括约肌间切除部分内括约肌治疗结肠息肉的临床研究

摘　　要：目的 评价腹腔镜下经括约肌间切除部分内括约肌治疗结肠息肉的可行性和安全性.方法 回顾性分析2012年3月～2013年12月共7例FAP患者的临床资料.结果 术后吻合口离肛缘1.8～3.2 cm,肛管无息肉残留,术中出血30～150 mL.本组7例均随访至2013年12月,排便次数平均3～8次/d,肛门无漏便和漏气,手术后短期内排便次数较多,随着时间推移,术后一年控便功能有明显改善.结论 采用腹腔镜全结直肠切除、经盆腔途径经括约肌间切除部分内括约肌、S型储袋回肠-肛管吻合治疗FAP技术上是可行的,短期疗效好.Objective To evaluate the feasibility and safety of laparoscopic partial internal anal sphincter technique for treatment of colorectal polyposis intersphincteric resection. Methods Retrospectively analysis the clinical data of 7 cases of patients with FAP from March 2012 to December 2013. Results Postoperative anastomotic 1.8-3.2 cm from anus, anal no polyp residue, intraoperative bleeding was 30-150 mL. The group of 7 cases were followed up to 2013 Decem- ber, defecation times average 3-8 times/d, no anal leakage and leakage, operation in short term after defecation times more, with the passage of time, a year after the control function was improved. Conclusion Laparoscopic colorectal operation,laparoscopic total proctocolectomy, pelvic way intersphincteric resection of internal sphincter, spouch ileal anal anastomosis, the operation in the treatment of FAP patients,radical is technically feasible, and short-term cura- tive effect.
