摘  要:目的探讨高迁移率族蛋白1(high mobility group box 1protein,HMGB1)和细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)在缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)新生儿血浆中的表达水平及其与新生儿神经行为(neonatal behavioral neurological assessment,NBNA)评分的相关性。方法收集30例正常新生儿和30例HIE患儿血液标本,分离血浆,采用酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)检测血浆HMGB1和ICAM-1浓度。并由受过专业培训的新生儿医师对其进行NBNA评分。结果与正常对照组相比,HIE患儿血浆ICAM-1和HMGB1水平显著升高(P<0.01);HIE患儿血浆HMGB1与ICAM-1呈正相关(r=0.854,P<0.01),而ICAM-1、HMGB1水平与NBNA评分呈负相关(分别为r=-0.851,P<0.01;r=-0.871,P<0.01)。结论 HIE患儿血浆HMGB1、ICAM-1浓度较非HIE新生儿明显升高且与HIE病情轻重密切相关,提示HMGB1、ICAM-1可能在新生儿HIE的发生发展中发挥重要作用。Objective To explore the expression and correlation of high mobility group box 1 protein (HMGB1),intercellular adhesion molecule-1 (ICAM-1) and neonatal behavioral neurological assessment (NBNA) in hypoxic-ischemic encephalopathy (HIE) neonates.Methods Thirty controls and 30 HIE neonates were recruited in our study.Plasma was prepared for detecting the HMGB1,ICAM-1 concentrations using ELISA kit.The NBNA score was determined by a specially trained physician.Results Comparing with controls,ICAM-1,and HMGB1 concentrations in plasma were significantly increased (P<0.01).Furthermore,there was a positive correlation between HMGB1 and ICAM-1 in HIE group (r=0.854,P<0.01).ICAM-1,HMGB1 levels and NBNA score showed significant negative correlations respectively (r=-0.851,P<0.01; r=-0.871,P<0.01) Conclusions The serum level of HMGB1,ICAM-1 in HIE patients are obviously higher than non-HIE neonates and are closely related to the severity of HIE,which may play an important role in the pathophysiology process of HIE.
