摘 要:目的 联合体积测量及磁敏感加权成像(SWI)技术观察忧郁型抑郁症及精神病型抑郁症的海马、杏仁核体积及其静脉系统变化,探究2组亚型的特征性影像学表现.方法 对18例忧郁型抑郁症、14例精神病型抑郁症患者及20例健康志愿者分别行海马、杏仁核的T1-MR形态扫描及SWI技术扫描,对比分析其体积及其静脉系统的变化规律.结果 2组亚型抑郁症患者的双侧海马体积均显著小于正常对照组,而2组亚型之间体积未见明显统计学意义;精神病型抑郁症的杏仁核体积显著小于忧郁型抑郁症及正常对照组,忧郁型抑郁症的双侧杏仁核体积与正常对照组相比无显著性差异;2组亚型抑郁症患者的侧脑室下静脉的直径、长度均显著小于正常组,而2组亚型之间无统计学意义.结论 杏仁核体积缩小可能是精神病型抑郁症的特征性影像学表现,此种改变或许可以作为鉴别2种抑郁亚型的一项较为客观的指标.抑郁症患者海马体积缩小与静脉萎缩之间可能有相关性.Objective To explore the imaging characteristics of the hippocampus,amygdala and the vein in the patients with mel- ancholic type depression and psychotic depression by the combination of MRI and SWI. Methods The hippocampus and amygdalas of 18 patients with melancholic type depresssion,14 patients with psychotic depression and 20 healthy volunteers were scanned by T1-MR and SWI. The hippocampal and amygdala sizes and the vein change among the three groups were analyzed. Results The sizes of hippocampus in the two major depression subtypes were smaller than that in healthy controls,while there was no statistical significance between the two major depression subtypes. Compared to healthy controls and patients with melancholic type depression, the size of amygdala in patients with psychotic depression was smaller, while there was no statistical significance between the patients with melancholic type depression and healthy controls. The diameter and length of the lateral ventricle veins in the two depression subtypes were smaller than that in healthy controls, while there was no statistical significance between the two depression subtypes. Conclusion The atrophy of the amygdala may he one imaging feature of the patients with psychotic depression , which can he an objective index to differentiate the two major depression subtypes. Moreover, the reduction of the bippocampus may be associated with intravenous atrophy.
