摘　　要：目的 探讨CT引导经皮穿刺植入放射性125I粒子治疗原发性肝细胞肝癌（HCC）并肝静脉、下腔静脉癌栓的可行性.方法 回顾分析本科10例行CT引导经皮穿刺植入放射性125I粒子治疗HCC合并肝右静脉、下腔静脉癌栓临床资料,术后2月行CT增强、肝功能、AFP检查.以后每2月复查.结果 所有患者均手术成功,无相关手术并发症出现.每位患者平均植入125I粒子10.2粒.术后2月,5例患者肝右静脉、下腔静脉内癌栓较前不同程度缩小,3例患者癌栓较前无明显变化,2例患者癌栓较前增大.结论 放射性125I粒子组织间植入治疗HCC合并肝静脉、下腔静脉癌栓是一安全有效的方法.Objective To assess the feasibility of CT-guided interstitial 12~I seed implantation for primary hepatic carcinoma with tumor thrombus in hepatic vein or inferior vena cava. Methods The data of ten cases with HCC and tumor thrombus in the right he- patic vein or inferior vena cava were retrospectively collected,all the patients were received CT-guided interstitial 125I seeds implanta- tion. 2 months after the operation, contrast CT, hepatic function and AFP were performed. Then every 2 months the patients were callback. Results All of the operations were successfully performed,there was no sever side-effect related to therapy. 10.2 125I seeds were implanted for per patient. 2 months after the treatment, the tumor thrombus in hepatic vein or inferior vena cava were decreased in5 cases, the tumor thrombus are not changed in 3 cases, while the tumor thrombus are increased in 2 cases. Conclusion 125I seeds implantation is a safe and effective therapy of HCC with tumor thrombus in hepatic vein and inferior vena cava.
