摘　　要：目的探讨肾上腺皮质黑色腺瘤的病理特征及临床特点。方法对1例肾上腺皮质黑色腺瘤的组织学、影像学、免疫组化及临床表现进行综合分析,结合文献讨论其临床病理特征、病理诊断与鉴别诊断。结果肿物位于肾上腺,大小2.5 cm×2.5 cm×2.2 cm,切面灰黑色,包膜完整,质软。镜下肿瘤细胞大小较一致,细胞境界清楚,排列成巢状、索状及腺泡状,少数细胞核增大、有轻度异型性但无核分裂。免疫组化Syn、vimentin、α-inhibin和CD56(+),CgA、S-100、EMA、CK(AE1/AE3)和melanA(-),Ki-67增组指数<1%。结论肾上腺皮质黑色腺瘤罕见,正确认识其临床病理特征可避免误诊或漏诊。Objective To investigate the clinical and pathological features of black adenoma of the adrenal cortex. Methods By analyzing histology, imaging, immunohistochemistry and clinical manifestations of one case of adrenocortical black adenoma, the clinicopathological features, diagnosis and differential diagnosis were discussed. Results The tumor lied in the adrenal and the size were 2. 5 cm ×2. 5 cm × 2. 2 cm, with grey black section and complete capsule, even soft texture. The tumor cells were more uniform in size with clear boundary and arranged in nests, cords and acinar under microscope. A few enlarged nuclei with mild atypia but no mitotic could be seen. Immunohistochemistry showed that the tumor cells were positive for vimentin, CD56, Syn and α-inhibin, but negative for CgA, S-100, EMA, CK （AE1/AE3）, MelanA, Ki-67 label index was less than 1%. Conclusion Adrenocortical black adenoma is rare, and a correct understanding of its clinical and pathological characteristics can avoid misdiagnosis or missed diagnosis.
