HCⅡ与LBC检测法联合应用在宫颈癌筛查诊断中的价值

摘　　要：目的探讨HPV DNA第二代杂交捕获(HCⅡ)检测法和液基细胞学(LBC)检测法及其联合应用在宫颈癌筛查诊断的价值。方法选取该院2013年3—10月门诊及体检中心就诊的患者500例,分别行LBC检测、HCⅡ法检测,对任一方法结果阳性者行阴道镜病理活检。结果 500例LBC异常占10.6%;HCⅡ异常占6.4%;HCⅡ+LBC异常占11.2%。106例检测阳性患者按要求做阴道镜病理活检,宫颈炎症、CINⅠ、CINⅡ、CINⅢ、宫颈癌发病率分别为74.5%、15.1%、6.6%、2.8%、0.9%;HCⅡ+LBC联合检测法阳性率分别为17.7%、56.2%、85.7%、100%、100%;单独的HCⅡ检测法阳性率分别为45.6%、62.5%、71.4%、100%、100%,随着病理恶性等级增高阳性率增高。结论 HCⅡ+LBC联合检测法与单独的HCⅡ检测法都是宫颈癌筛查的好方法。HCⅡ+LBC联合检测法是宫颈癌筛查的首选。但对于经济不发达的城乡地区的宫颈癌筛查也可采用单独的HCⅡ检测法,以便及早发现宫颈癌及时诊治。Objective To investigate the value in the diagnosis of cervical cancer screening by using the method of second -generation hybrid capture HPV DNA（HCⅡ） only and combined with liquid -based cytology（LBC）.Methods five hundred cases of patients were selected from March to October 2013 in Suzhou Municipal Hospital ,which underwent LBC detection and HCⅡassay respectively . Any patient with positive result for either method would have colposcopy biopsy .Results Among the 500 cases of patients ,LBC,HCⅡand HCⅡ ＋LBC abnormalities accounted for 10.6%,6.4%,11.2%respectively.A hundred and six patients had colposcopy biopsy . The incidence rates of inflammation of the cervix ,CINⅠ,CINⅡ,CINⅢ,cervical cancer were 74.5%,15.1%,6.6%,2.8%,0.9%respectively.HCⅡ ＋LBC positive rates of detection were 17.7%,56.2%,85.7%,100%,100% respectively.HCⅡ single positive rates were 45.6%,62.5%,71.4%,100%and 100%respectively,which increased with the malignant grade of pathology .Conclusions HCⅡ ＋LBC and HCⅡassays are good ways in cervical cancer screening .HCⅡ ＋LBC is the preferred method of cervical cancer screening .But for the economically underdeveloped rural and urban areas ,single HCⅡassay can also be use for early detection ,diagno-sis and treatment of cervical cancer .
