摘 要:目的探讨神经纤毛蛋白(NRP)-1、2在大肠癌组织中的表达及其与微血管密度(MVD)的相关性,以揭示NRP-1、2与肿瘤血管生成的关系。方法采用免疫组织化学方法检测66例大肠癌患者的癌组织、癌旁组织及正常组织中NRP-1、2的表达情况,以CD105标记肿瘤新生血管内皮细胞,计数MVD值。结果 (1)NRP-1、NRP-2在大肠癌组织、癌旁组织、正常组织中的表达阳性率分别为71.2%、25.8%、0,及80.3%、15.2%、0,各组间比较差异有统计学意义(P<0.01)。(2)MVD在大肠癌组织、癌旁组织及正常组织中的计数分别为35.682±5.542、14.485±3.301、6.864±1.771,各组相比差异有统计学意义(F=986.654,P<0.01)。(3)不同年龄、性别、肿瘤的部位及分化程度的患者NRP-1、2的表达差异无统计学意义。肿瘤>5 cm的患者NRP-1阳性表达率较高,肿瘤侵及浆膜及以外、有淋巴结转移的患者NRP-1、2的阳性表达率均较高,不同的Dukes分期患者NRP-1、2阳性表达差异亦有统计学意义(P<0.05)。(4)大肠癌患者不同性别、年龄及肿瘤大小、部位和组织分化程度MVD的表达差异无统计学意义,而浸润到浆膜及以外、有淋巴结转移、Dukes分期高的患者相关MVD的表达增加(P<0.01)。(5)大肠癌组织中NRP-1(r=0.599,P<0.01)、NRP-2(r=0.484,P<0.01)的表达与MVD计数均呈正相关关系。结论 NRP-1、2在大肠癌的血管新生中起重要作用,并且与大肠癌侵袭和转移关系密切。Objective To observe the expression of Neuropilin (NRP)- 1, 2, to calculate micro vessel density (MVD) in colorectal cancer, paraneoplastic tissues, normal tissues, and to investigate the correlation between NRP-1, 2 and MVD to understand the role of NRP- 1, 2 in the process of tumor angiogenesis. Methods Expression of NRP- 1 and NRP-2 were studied by immunohistochemistry in 66 specimens from colorectal cancer, paraneoplastic tissues and normal tissues. MVD was assessed based on CD105 immunohistochical staining. Results (1) The positive expression of NRP-1, 2 in colorectal cancer, paraneoplastic tissues, normal tissues were 71.2%, 25.8%, O; 80.3%, 15.2%, 0 respectively. There was a statistical difference between them (P 〈 0.05). (2)The quantity of MVD in colorectal cancer, paraneoplastic tissues and normal tissues were 35.682±5.542, 14.485±3.301 and 6.864±1.771 respectively. There was a statistical difference between them (P 〈 0.05). (3) Expression of NRP- 1, 2 had a good correlation with the tumor size, infiltrating depth, lymph node metastasis, Dukes stag- ing (P 〈 0.05), but no correlation was found with the tumor position, histological types, differentiation degree and the gender or age of patients (P 〉 0.05). (4) The MVD value had no correlation with the gender or age of patients, tumor size, position, histological types (P 〉 0.05), but had a good correlation with infiltrating depth, lymphatic metastasis and Dukes staging (P 〈 0.05). (5) There was a positive correlation between NRP-1, 2 and MVD value (P 〈 0.01). Conclusion NRP-1, 2 may play an important role in the angiogenesis of the colorectal cancer, and it correlates with the invasion and metastasis of colorectal cancer closely.
