改良折叠型人工晶状体缝线固定术治疗眼外伤晶状体玻璃体切除无晶状体眼

摘　　要：目的探讨利用改良方式人工晶状体植入系统行二期折叠型人工晶状体缝线固定术，治疗眼外伤晶状体玻璃体切除术后无晶状体眼的手术技巧及疗效。方法对2011年5月至2012年12月在唐山市眼科医院收治的21例（21只眼）因眼外伤而行晶状体、玻璃体切除术后无晶状体眼，应用改良方法行二期折叠型人工晶状体缝线固定术患者进行回顾性分析，探讨手术技巧、疗效及术后并发症。术后随访3-6个月，平均（4.4±0.5）个月。结果21只眼一次悬吊成功。手术时间明显缩短，术中未发生明显手术并发症，术后1只眼发生脉络膜脱离，1只眼眼压升高。术后第1天，15只眼的裸眼视力为0.3～0.8，占71.43％（15／21），术后1周、3、6个月裸眼视力0.3～0.8者分别为17只眼（80.95％，17／21），19只眼（90.48％，19／21）和20只眼（95.24％，20／21）。术后3个月裸眼视力较术前最佳矫正视力，差异无统计学意义（t=1.0639，P=0.2936）。同一术者改良前、后行折叠型人工晶状体缝线固定术所用平均时间分别为（90.5600±7.2404）min和（69.6190±3.9934）min，手术时间差异比较有统计学意义（t=11.8166，P〈0.01）。结论眼外伤晶状体、玻璃体切除术后应用改良方法使用人工晶状体植入系统行二期折叠人工晶状体缝线固定术，手术时间缩短，手术效果可靠，术中术后并发症少，是治疗眼外伤晶状体、玻璃体切除术后无晶状体眼屈光矫正的理想手术方法。Objective To examine the operative technique and its curative effect of using modified operation with Iol Delivery System secondary foldable intraocular lens with suture fixation to treat aphakic eye after ocular trauma with lens-vitrectomy. Methods Through the retrospective analysis of the 21 patients （21eyes） who was used modified operation to implant foldable intraocular lens secondarily with suture fixation at ciliary sulcus to examine the operative effect, technique and postoperative complication. The postoperative follow-up period was 3-6 months, averaged （4.4±0.5） months. Results All 21 eyes had successful operation at one time. Operation time was shortened obviously and no obvious complication was occurred intraoperatively. Choroidal detachment was observed in l eye and ocular hypertension was occurred in 1 eye postoperatively. Uncorrected visual acuity 0.3-0.8 was achieved in 15 eyes （71.43%, 15/21） on the first day postoperatively. Uncorrected visual acuity 0.3-0.8 was achieved in 17 eyes （80.95%, 17/21）, 19 eyes （90.48%, 19/21）, and 20 eyes （95.24%, 20/21） at the 1 week, 3 and 6 months follow-up respectively. There was not clinical significant difference between visual acuity of preoperative and post-operative （t =1.0639, P =0.2936）. Average operation time was （90.5600±7.2404） minutes and （69.6190±3.9934） minutes before and after the improvement. There was clinical significant difference by using modified ooeration ,（t =11.8166, P 〈0.001）. Conclusions Using modified operation with Iol Delivery System secondary foldable intraocular implantation with suture fixation at ciliary sulcus is successful to correct aphakic eye after ocular trauma with lens-vitrectomy.
