广西成人骨性Ⅲ类错牙合畸形特征及侧貌影响因素分析

摘　　要：目的:通过研究广西地区成人骨性Ⅲ类错牙合畸形软硬组织侧貌特征及软硬组织的相关性,为正畸-正颌联合治疗骨性Ⅲ类错牙合畸形提供参考。方法:选择67例骨性Ⅲ类错牙合成人患者及56名面型较佳骨性Ⅰ类成人患者,选取能充分体现软硬组织侧貌表面形态特征的21项指标,进行对照分析和对骨性Ⅲ类错牙合患者软硬组织进行相关性、线性回归分析。结果:与Ⅰ类组相比,Ⅲ类组除ANS-Me/N-Me(%)、L1-NB(mm)、MP-SN(°)外,其余18项指标差异均有统计学意义;多项硬组织指标与其对应软组织指标表现出高相关性,其中FH-N`Pg`与NP-FH、Y轴角相关系数分别高达0.965、-0.908。结论:与Ⅰ类错牙合相比,Ⅲ类错牙合组侧貌特征主要表现在面下1/3的矢状向发育异常,垂直向无明显异常。影响骨性Ⅲ类错牙合患者软组织侧貌特征的因素为骨性、牙性、软组织厚度3因素,其中骨性因素最为明显。Objective To provide reference for the orthodontic-orthognathic surgical treatment of patients with skeletal class III malocclusion by studying the correlation between the charateristics of soft and hard tissue profile,features of soft and hard tissue of patients with skeletal class III malocclusion in Guangxi Province. Methods The study included 67 skeletal class III malocclusion adult patients and 56 skeletal class I malocclusion adult patients.Twenty-one indexes pertaining to the charateristics of soft and hard tissue profile were chosen.Contrast analysis, correlation analysis and linear regression analysis of these indexes between the two groups of patients were then conducted. Results There were significant differences between the two groups in the above mentioned indexes,except for ANS-Me/N-Me （%）,L1-NB （mm） and MP-SN （~）.High correlation was found in several hard tissue indexes and their corresponding soft tissue indexes, the correlation coefficient of NP-FH and Y axial angle and FH-Pg attain to 0.965 and -0.908 respectively. Conclusion Anteroposterior abnormal growth of lower anterior face is the main feature of facial profile of patients with skeletal class III malocclusion.Osseous factor, dental factor and soft tissue thickness are three main influencing factors affect facial profile of patients with skeletal class III malocclusion.
