乳腺癌超声引导下空芯针活检与术后病理学检查的对照研究

摘　　要：目的：评价超声引导下空芯针穿刺活检与术后病理学检查在乳腺癌病理学诊断及癌组织免疫组化（IHC）检测ER、PR、HER-2结果的符合率。方法收集2008年10月～2009年6月于辽宁省肿瘤医院乳腺外科住院的乳腺癌患者85例。术前由专业的、有经验的医生在超声引导下空芯针活检，活检后由固定的、有经验的病理医师进行固定、染色和读片。再对组织病理行组织类型、组织分级、ER、PR和C-erbB-2的检测，并与根治性手术后标本病理学检查结果对照，评价其符合率。结果超声引导空芯针穿刺活检与术后病理学检查在组织类型、组织分级、ER、PR和C-erbB-2方面的符合率分别为：88．2％、100％、96．5％、92．9％及97．6％。结论通过加强超声引导空芯针穿刺活检，以及穿刺后标本制片、读片的质量控制，空芯针活检可以准确获得乳腺癌组织类型、组织分级、ER、PR、C-erbB-2的情况，进而指导临床治疗。Objective To evaluate the performance of the ultrasound -guided core needle biop-sy in the diagnosis of breast cancer with that of postoperative pathological diagnosis （ ER, PR, HER-2） .Methods A total of 85 breast cancer patients who were treated in Liaoning Cancer Hos -pital from October 2008 to June 2009 were enrolled in the study.Ultrasound-guided core needle bi-opsy was performed by a professional experienced doctor before the operation , and the specimens were fixed, stained and read by an experienced pathologist.The coincidence rate of the pathological tissue type, histological grade, ER, PR and C-erbB-2 was compared between the ultrasound -guided core needle biopsy and postoperative pathological examination .Results The coincidence rate of pathological tissue type, histological grade, ER, PR and C-erbB-2 was 88.2 %, 100 %, 96.5 %, 92.9% and 97.6 %, respectively, between ultrasound-guided core needle biopsy and postoperative pathological examination.Conclusions Ultrasound-guided core needle biopsy can ac-curately obtain the information about cancer tissue type , histological grade, ER, PR, C-erbB-2, and guide the clinical treatment of breast cancer.
