降下唇肌瓣与胸锁乳突肌瓣联合修复口底全层软组织缺损

摘　　要：目的：探讨应用降下唇肌瓣和胸锁乳突肌瓣联合修复口底全层软组织缺损的方法和效果。方法 ：2010年5月—2013年9月间,对5例口底恶性肿瘤患者行肿瘤扩大切除术,遗留的口底全层软组织缺损采用降下唇肌瓣和胸锁乳突肌瓣联合修复。通过观察组织瓣成活、伤口愈合、舌活动度及唇和颏部外形等情况,评价其修复效果。结果：随访3-15个月,组织瓣全部成活,未出现伤口感染和口颈瘘,舌活动良好,唇和颏部外形良好。结论：降下唇肌瓣和胸锁乳突肌瓣制备简便,创伤小,两者联合应用是口底全层软组织缺损的理想修复方法。PURPOSE：To investigate the method and efficacy of reconstruction of full-thickness soft tissue defects of floor of mouth by combining depressor labii inferioris flap with sternomastoid muscle flap. METHODS： Between May2010 and September 2013, five patients with cancer in floor of mouth underwent tumor resection, and the full-thickness soft tissue defects were simultaneously reconstructed with depressor labii inferioris flap and sternomastoid muscle flap. The patients were followed up for 3-15 months to evaluate the flap survival, mobility of tongue and esthetic outcome of lip and mental region. RESULTS： All the flaps survived completely, the tongue moved well, and functional and esthetic results were good,no wound infection and cervical fistula occurred. CONCLUSION： Combining depressor labii inferioris flap with sternomastoid muscle flap are suitable for repairing full-thickness soft tissue defects of floor of mouth.
