摘  要:目的构建骨缺损诱导膜模型,探讨骨诱导膜对灭活移植骨成骨诱导活性的作用。方法 32只新西兰大白兔一期在桡骨中段制造骨缺损模型,植入聚甲基丙烯酸甲酯骨水泥诱导生物膜形成。第6、8周分别处死4只取出诱导膜检测骨形态发生蛋白(bone morphogenetic protein-2,BMP-2)和血管内皮生长因子(vascular endothelial growth factor,VEGF)含量并切片作HE染色,取同只兔皮下组织作为对照。其余24只兔在一期植入骨水泥后6周取出骨水泥进行二期手术,按随机数字表法分为实验组、模型组和对照组,每组8只。实验组保留诱导膜并在膜内植入灭活骨,模型组保留诱导膜不植骨,对照组去除诱导膜植入灭活骨。二期处理后第8、12周通过放射学和组织学评估新生骨形成情况。结果ELISA检测表明在第6、8周诱导膜中BMP-2含量[(437.40±20.75)、(346.67±30.05)pg/mL]、VEGF含量[(362.33±38.85)、(233.84±18.20)pg/mL]均高于皮下组织(P<0.05),同时在诱导膜切片HE染色中观察到软骨内骨化;放射学检测可见二期处理后第8、12周实验组成骨优于模型组和对照组(P<0.01),模型组有少许骨痂形成,对照组无成骨;第8、12周组织学观察实验组可见骨细胞和软骨细胞形成,成骨优于模型组和对照组(P<0.01),对照组仅有纤维连接,未见骨细胞和软骨细胞。结论骨诱导膜含有间充质干细胞和高浓度的细胞因子,从而促进灭活移植骨重建骨缺损。Objective To construct a bone defect-induced membrane model and to discuss the osteogenic activity of the bone induced membrane on inactivated bone graft.Methods A radial defect model was built with 32 New Zealand rabbits.Polymethyl methacrylate( PMMA) bone cement spacer was inserted into the defect to induce membrane.After 6 and 8 weeks,8 rabbits( 4 for each time) were sacrificed and bone induced membranes were taken out to test bone morphogenetic protein-2( BMP-2) and vascular endothelial growth factor( VEGF) levels and to perform HE staining of sections,with rabbit subcutaneous tissue as control.Bone cement was taken out from the other 24 rabbits in 6 weeks after implantation for secondary operation,including 3 groups,an experimental group [keeping the induced membrane in situ and implanting autologous iliac inactivated with high temperature water( 100 ℃ 30 min) ],a model group( keeping the induced membrane in situ),and a control group( removing the induced membrane and implanting autologous iliac inactivated with high temperature water).Radiological and histological evaluation of the new bone formation was done at 8 and 12 weeks.Results( 1) BMP-2 and VEGF levels in the induced membrane were 437.40 ±20.75 pg /mL and 362.33 ± 38.85 pg /mL,respectively,at 6 weeks and 346.67 ± 30.05 pg /mL and233.84 ± 18.20 pg /mL,respectively,at 8 weeks,higher than those of the subcutaneous tissue( P〈0.05),while endochondral ossification was observed in induced membrane slice.( 2) Radiological detection at 8 and 12 weeks after the secondary operation indicated that bone formation was more in the experimental group than in the model group and the control group( P〈0.01).There was a little callus formation in the model group,while the control group had no bone formation.( 3) Histological detection at 8 and 12 weeks after the secondary operation indicated that bone formation was more in the experimental group than in the model group and the control group( P〈0.01).Only f
