摘 要:目的比较远视眼与近视眼角膜生物力学参数和眼压的差异。方法分别选取远视眼与近视眼各60眼,应用眼反应分析仪测量角膜生物力学的指标:角膜滞后量和角膜阻力因子量,模拟Goldmann眼压,角膜补偿眼压。对远视组和近视组的角膜屈光力、中央角膜厚度及角膜生物力学相关参数进行t检验;对角膜生物力学特性的影响因素进行相关性分析;对中央角膜厚度与眼压的关系进行Pearson相关分析。结果远视组与近视组之间角膜屈光力差异有显著统计学意义(t=-4.103,P<0.01)。仅中央角膜厚度与角膜滞后量和角膜阻力因子量呈显著正相关(r=0.637、0.519,均为P<0.01)。中央角膜厚度与非接触眼压计测得的眼压和模拟Goldmann眼压均呈正相关(r=0.428、0.406,均为P<0.01),与角膜补偿眼压不相关(r=0.075,P=0.466)。角膜屈光力与非接触眼压计测得的眼压呈正相关(r=0.283,P<0.01),与模拟Goldmann眼压和角膜补偿眼压无相关性(r=0.186、0.117,P=0.067、0.251)。结论角膜的生物力学参数主要与角膜厚度呈正相关,不受眼屈光状态影响。Objective To compare the differences of corneal biomechanics and intraocular pressure between hyperopia and myopia eyes. Methods Corneal hysteresis, corneal resistance factor, Goldmann correlated intraocular pressure value and corneal compensated intraocular pressure were measure by the ocular response analyzer in 60 myopia eyes and 60 hyperopia eyes, respectively. The t-test was used to compare the central corneal thickness, corneal refractive power, and corneal biomechanical parameters between the myopia and hyperopia groups. Pearson correlated analysis was used to explain the impact of corneal biomechanical effective factors and the relationship of central corneal thickness and intraocular pressure. Results Corneal refractive power was significantly different between myopia and hyperopia groups (t = -4. 103, P 〈 0. 01 ). The central corneal thickness alone was positively correlated with the corneal hysteresis and corneal resistance factor ( r = 0. 637 ,0. 519 , all P 〈 0.01 ). Central corneal thickness was positively correlated with the intraocular pressure measured by non-contact tonometer and the Goldmann correlated intraocular pressure value ( r = 0. 428, 0. 406,all P 〈 0.01 )and was not correlated with the corneal compensated intraocular pressure ( r = 0.075 ,P = 0.456). Corneal refractive power was slightly positively correla- ted with the intraocular pressure measured by non-contact tonometer ( r = 0.283, P 〈 0. 01 )and was not correlated with the Goldmann correlated intraocular pressure and corneal compensated intraocular pressure(r =0. 186,0. 117;P =0.067,0. 251 ). Conclusion Corneal biomechamcal parameters are mainly affected by central corneal thickness, and are not correlated with the refractive condition of eyes.
