差异表达基因巨噬细胞移动抑制因子及相关因子在早期宫颈鳞癌组织中的表达及临床意义

摘  要:目的检测早期宫颈鳞癌基因芯片差异表达基因——巨噬细胞移动抑制因子(MIF)、白细胞介素(IL)-8与基质金属蛋白酶(MMP)-9在早期宫颈鳞癌中的表达及相互关系,探讨这些因子在早期宫颈鳞癌淋巴转移中的作用机制。方法采用反转录-聚合酶链反应(RT-PCR)检测63例无淋巴结转移,23例有淋巴结转移早期宫颈鳞癌组织中MIF、IL-8、MMP-9的mRNA表达水平;收集126例新鲜及归档早期宫颈鳞癌组织蜡块制作组织芯片,采用免疫组织化学方法检测MIF、IL-8、MMP-9蛋白的表达。结果 MIF、IL-8、MMP-9 mRNA和蛋白水平的表达在有淋巴结转移的早期宫颈鳞癌组织中显著高于无转移组织(P<0.01),各因子的表达均与早期宫颈鳞癌的淋巴结转移呈正相关(r=0.34;r=0.48;r=0.495)。MIF和IL-8、MMP-9之间均有等级相关性(r=0.272;r=0.54);IL-8与MMP-9的表达呈正相关(r=0.409)。结论 MIF、IL-8、MMP-9在早期宫颈鳞癌中高表达,且与淋巴结转移密切相关,提示这些因子可能参与早期宫颈鳞癌的淋巴转移;MIF可能是调控IL-8和MMP-9表达的上游因子。Objective To determine the expressions and interactions of macrophage migration-inhibitory factor (MIF), interleukin(IL)-8 and matrix metalloproteinase(MMP)-9 in cervical squamous cell carcinoma, and to investigate the mechanisms of the factors associated with the lymph node metastasis. Methods MIF, IL-8 and MMP-9 mRNA was quantified by reverse transcription polymerase chain reaction in 23 patients with cervical squamous cell carcino-ma who had lymph node metastasis and 63 patients without lymph node metastasis. One hundred and twenty-six pieces of newly biopsied tissues were employed to prepare the parafin blocks, which entailed detection of the expres-sions of MIF, IL-8 and MMP-9 via immunohistochemistry assay. Results The mRNA and protein expressions of MIF, IL-8 and MMP-9 were consistently higher in patients with cervical cancer who had lymph node metastasis than those without (P〈0.01). The expression levels of MIF, IL-8 and MMP-9 in cervical cancer were positively asociated with lymph node metastasis (r=0.34, 0.48, 0.495). There was a positive correlation between MIF and IL-8 and MMP-9 re-spectively (r=0.272, 0.54), and between IL-8 and MMP-9 (r=0.409). Conclusion The fact that MIF, IL-8 and MMP-9 are highly expressed in early-stage cervical cancer tissues and that these markers are closely related to lymph node metastasis suggest that these markers might have contributed to the tumorigenesis of early-stage cervical cancer. MIF might be the upstream factor which regulates the expression of IL-8 and MMP-9.
