非球面人工晶体植入术后全眼球差的可预测性分析

摘　　要：目的探讨超声乳化白内障吸除术后植入不同球差的非球面人工晶体(IOL)后全眼球差的可预测性。方法收集2013年1—5月拟行手术治疗的年龄相关性白内障58例(58眼),随机分为3组,行超声乳化白内障吸除并分别植入非球面IOL。Tecnis组20例植入Tecnis Z9003,IQ组19例植入ArcySof IQ,PY-60AD组19例植入PY-60AD。术后3个月观察患者的裸眼视力、最佳矫正视力、等效球镜值、6.0 mm瞳孔直径时的角膜及全眼球差。结果术后3个月3组患者的裸眼视力、最佳矫正视力、等效球镜值、手术诱导的角膜球差、预测误差差异均无统计学意义(P>0.05);3组术前、后角膜球差比较差异均无统计学意义(P>0.05);3组全眼球差的预测值与测量值差异均无统计学意义(P>0.05)。结论超声乳化白内障吸除术后植入不同球差的非球面IOL,术后假晶体眼全眼球差的可预测性较好,为非球面IOL的个性化植入提供了理论依据。Objective To investigate the predictability of the total ocular spherical aberration in pseudophakic eyes with different aspheric intraocular lenses （ IOL） implantation after phacoemulsification. Methods Fifty-eight pa-tients with age-related cataract （58 eyes） from January to May 2013 were randomly divided into three groups to undergo phacoemulsification and aspheric IOL implantation respectively. The Tecnis group （20 eyes） was implanted with Tecnis Z9003 IOL;IQ group （19 eyes） was implanted with ArcySof IQ IOL;and PY-60AD group （19 eyes） was implanted with PY-60AD IOL. The uncorrected visual acuity （UCVA）, best corrected visual acuity （BCVA）, spherical equivalent re-fraction, spherical aberration of the cornea and whole eye at a 6-mm pupil were observed 3 months after the operation. Results There were no significant differences in UCVA, BCVA, spherical equivalent refraction, surgically induced cor-neal spherical aberration and predictive error 3 months after the operation among the three groups （P〉0. 05）;The differ-ences between the preoperative and postoperative corneal spherical aberration in the three groups were not statistically sig-nificant （P〉0. 05）;the differences between the predicted value and measured value of postoperative total spherical ab-erration in the three groups were not statistically significant （P〉0. 05）. Conclusion The predictability of the total ocu-lar spherical aberration in pseudophakic eyes with different aspheric IOL implantations after phacoemulsification is favora-ble, and it may provide basis for individuation implantation of the aspheric IOLs.
