摘　　要：目的观察强化他汀治疗在症状性颅内动脉狭窄中的疗效。方法选择120例症状性颅内动脉狭窄患者,采用随机表法分为A组(阿托伐他汀10mg/d)、B组(20mg/d)和C组(40mg/d)各40例,治疗至少持续12个月,随访12个月,观察临床终点事件发生率及治疗前后脑血流灌注值的变化,评价强化调脂疗效。结果 3组在治疗后12个月随访时,A组、B组和C组脑血管终点事件发生率分别为26.3%、13.5%和5.4%;A组与C组比较,差异有统计学意义(P<0.05)。与治疗前比较,治疗后12个月,3组相对脑血流量和相对脑血容量明显升高,差异有统计学意义(P<0.05),C组相对达峰时间明显缩短,差异有统计学意义(P<0.05)。结论持续12个月的强化调脂治疗(40mg/d)可以减少脑血管事件的发生,提高缺血区脑组织血流灌注。Objective To observe the effect of intensive statin therapy on symptomatic intracranial artery stenosis （ICAS） .Methods One hundred and twenty symptomatic ICAS patients were ran-domly divided into group A ,group B and group C （40 in each group） .The patients in 3 groups were treated with atorvastatin at the dose of 10 mg/d ,20 mg/d ,and 40 mg/d ,respectively ,for at least 12 months .The patients were followed up for 12 months ,during which the incidence of car-diovascular events and perfusion of ischemic brain tissues were observed .The effect of intensive statin therapy on symptomatic ICAS was assessed .Results The incidence of cardiovascular events was 26 .3% ,13 .5% and 5 .4% ,respectively ,in groups A-C after 12 months of atorvastatin treatment ,and significantly higher in group A than in group C （26 .3% vs 5 .4% ,P〈0 .05） .The rCBF and rCBV were significantly higher in groups A-C after treatment than before treatment （P〈0 .05） .The rTTP was shorter in group C than in groups A and B （P〈0 .05） .Conclusion Twelve-month intensive statin therapy （40 mg/d） can reduce the cerebrovascular events and im-prove the perfusion of ischemic brain tissues .
