摘  要:目的:探讨大、小动脉病变对急性缺血性中风后认知功能的影响。方法:选取急性缺血性中风患者60例,根据类肝素药物治疗急性缺血性脑卒中试验分型诊断标准结合影像学的方法,分为大动脉病变组17例,小动脉病变组43例,于发病后90d使用蒙特利尔认知评价量表MoCA对患者的认知功能进行评价,对两组患者认知功能损伤程度进行比较。结果:大动脉病变组病灶在皮层的分布多于小动脉病变组(P<0.05),在桥脑的分布少于小动脉病变组(P<0.05);在额叶的分布多于小动脉病变组(P<0.05)。大动脉病变组的MoCA总分和命名、注意、语言、抽象4个分项的得分均低于小动脉病变组(P<0.05,P<0.01)。结论:大动脉病变对急性缺血性中风后认知功能的损伤程度较小动脉病变重。Objective: To investigate the impact of large and small artery lesions on cognitive function after acute ischemic stroke. Methods: 60 cases of patients with acute ischemic stroke were collected and divided into large artery lesion group and small artery lesion group based on the TOAST classification diagnosis criteria and iconography data, with 17 cases in large artery lesion group and 43 cases in small artery lesion group. Cognitive function of two groups was evaluated and compared with each other by MoCA scale at 90 days after the stroke. Results: The numbers of ischemic area in cortex and frontal lobe of large artery lesion group are more than small artery lesion group (P〈0.05), but in pons the result is contrary (P〈0.05). The MoCA scores of naming, attention, language and abstraction in large artery lesion group were lower than those in small artery lesion group (P〈0.05, P〈0.05). Conclusion: The cognitive impairment of large artery lesion after acute ischemic stroke is much severer than that of small artery lesion.
