摘 要:目的观察脑梗死患者血浆叶酸和维生素B12(VitB12)与同型半胱氨酸(Hcy)的相关性。方法检测243例脑梗死患者血浆叶酸、VitB12、Hcy水平,并根据叶酸水平将Hcy分为低于正常组(A组)、正常偏低组(B组)和偏高组(C组),比较叶酸与Hcy的相关性;同样,根据VitB12水平再将Hcy分为A、B、C三组,比较两者相关性。结果叶酸A、B、C三组之间Hcy水平两两比较均差异有统计学意义(P<0.05),同样,VitB12三组比较均差异有统计学意义(P<0.05)。Hcy与叶酸和VitB12相关系数rs分别为-0.560,P=0.00和-0.302,P=0.00。149例高Hcy(>16 mmol/L)患者中,叶酸和VitB12水平都在A组和B组的占73%(108例),都在C组的占4%(6例)。叶酸水平在A组和B组,而VitB12在C组的占11%(17例);VitB12水平在A组和B组,而叶酸在C组的占12%(18例)。结论叶酸或VitB12的绝对缺乏或相对缺乏是高Hcy血症的重要原因。叶酸和VitB12水平与Hcy水平均呈负相关,且与叶酸的相关性强于VitB12,但在部分人群中其相关性可能存在差异。Objective To study the correlation of level of plasma folic acid and vitamin B12(VitB12) with homocysteine(Hcy) in patients with cerebral infarction.Methods Plasma concentrations of homocysteine,folic acid and vitamin B12 of 243 patients with cerebral infarction were detected.And the patients were divided into 3 groups according to the level of folic acid:the below normal group(group A),the normal low group(group B) and the normal high group(group C) respectively,then,the correlation between folic acid and Hcy was analyzed.Simmilarly,patients were divided into 3 corresponding groups according to the level of VitB12,and then the correlation between VitB12 and Hcy was analyzed.Results The levels of Hcy in 3 folic acid groups were compared pairwise,the differences were statistically significant;and the levels of Hcy in 3 VitB12 groups were compared pairwise,and the differences were statistically significant(P < 0.05).The correlation coefficient of Hcy and folic acid,VitB12 was-0.560,P =0.00 and-0.302,P =0.00 respectively.Among the 149 patients of hyperhomocysteinemia (Hcy > 16 mmol/L),the proportion of A and B group members in terms of both folic acid and VitB12 was 73 % (108 cases),and the proportion of C group members in terms of both folic acid and VitB12 was 4% (6 cases).Patients those levels of folic acid belonged to group A and B while levels of VitB12 belonged to group C accounted for 11% (17 cases).Furthermore,patients those levels of folic acid belonged to group C while levels of VitB12 belonged to group A and B accounted for 12%(18 cases).Conclusion Absolute or relative deficiency of folic acid or VitB12 is an important factor that causes hyperhomocysteinemia.levels of folic acid and VitB12 were negatively correlated with Hcy level,while the correlation between the level of folic acid and Hcy level were more significant.However,in some cases,the correlation of the plasma folic acid and VitB12 with Hcy may vary from people to people.
