化疗联合奥施康定在晚期胰腺癌癌痛中的作用

摘　　要：目的比较吉西他滨、顺铂姑息性化疗联合奥施康定和单用奥施康定控制晚期胰腺癌癌痛的作用。方法对42例晚期胰腺癌患者分成两组。一组用吉西他滨、顺铂姑息性化疗联合奥施康定(联合组22例)控制癌痛,另一组单用奥施康定(单药组20例)控制癌痛,并比较两组疗效差异。结果研究表明联合组镇痛有效率为95%(21/22),单药组镇痛效有效率为70%(14/20)。并且CBR方面,联合组较单药组平均减少镇痛药用量50%,平均KPS体力改善提高50%,平均体重增加幅度提高75%。结论联合组比单药组在治疗晚期胰腺癌癌痛、改善患者生活质量方面有更佳的疗效。Objective To compare the clinic effect between palliative chemotherapy（Gemcitabine and Cisplatin） combined with Oxycontin and Oxycontin alone for controlling advanced pancreatic cancer pain. Methods 42 cases with advanced pancreatic cancer were divided into two groups. For controlling the cancer pain, 22 cases in the combination group were treated with palliative chemotherapy（Gemcitabine and Cisplatin） and Oxycontin and 20 cases in the single drug group were treated with Oxycontin alone.The efficacy of the two groups was compared. Results The study showed that the effective rate of analgesia in the combination group was 95%（21/22）, and that in the control group was 70%（14/20）. And in the aspect of clinical benefit response（CBR）, the dosage of analgesics used in the combination group was half of that used in the control group, but the average KPS physical improvement increased by 50%, and the average weight increased by 75%. Conclusion The combination group has better efficacy in the aspects of treating the advanced pancreatic cancer pain as well as improving the quality of life of the patients compared with the single drug group.
