巯基乙酸酯培养基在白内障术前结膜囊感染细菌筛查结果中的评价

摘　　要：目的通过对比使用巯基乙酸酯(Th)培养基及增菌液对白内障患者术前球结膜囊标本进行细菌培养,探索应用Th培养基对培养结果的影响及其临床应用可行性,并评价临床所用消毒剂对眼部细菌的杀灭效果。方法将白内障患者术前3个时期的球结膜囊分泌物分别同时接种于临床常规应用培养基及Th培养基中,在同等细菌培养条件下,对培养结果进行观察及细菌鉴定。结果入院时、消毒前(滴加替沙星滴眼液)及消毒后(5%聚维酮碘消毒)细菌培养的阳性率分别为:79.4%、41.6%和3.3%,培养阳性率明显逐渐降低。增菌液培养出的细菌种类为9种,检出菌株的阳性率为56.3%,而Th培养基培养出细菌16种,阳性率为100%。结论临床应用Th培养基代替常规的增菌液,可增强细菌尤其是厌氧菌检出的阳性率。临床眼科所用的消毒剂5%聚维酮碘,对眼部细菌的杀灭效果显著,符合手术要求。Objective To determine the influence of thiolglycolate（Th）medium for conjunctival sac bacterial infection and to evaluate the probability of replacement for the routine enrichment broth method and the clinical effect of disinfectants used in the eyes.Methods To collect the specimens from conjunctival sac with sterile cotton swab in 3times before surgery,and then cultured in general medium and Th medium respectively,for isolation and identification under the same condition.Results The bacterial isolation rate was 79.4%,41.6% and 3.3%respectively in un-disinfected,prior to disinfection and after disinfection.The species of bacteria isolated from enrichment broth were 9,and isolation rate was 56.3%;on the other hand,the species of bacteria isolated from Th medium were 16,and the isolation rate was 100%.Conclusion The application of Th medium insteading of the conventional medium for the clinical bacterial culture can increase the isolation rate of bacterial culture,particularly anaerobic bacteria isolation.In addition,disinfectants used in clinical ophthalmology,such as 5% povidone-ildine,have significant effects on eyes＇ disinfection which is in accord with the surgical requirements.
