全数字化乳腺钼靶立体定位在老年隐匿性乳腺病变诊断治疗中的应用

摘　　要：目的探讨全数字化乳腺钼靶X线立体定位真空辅助旋切和立体定位空芯针穿刺活检术（SCNB）对老年隐匿性乳腺病变的诊断及治疗价值。方法选取2012年1月-2013年6月在我院诊治的60例临床体检阴性而影像学异常的老年隐匿性乳腺病变患者,其中38例存在可疑钙化病灶患者行乳腺X线立体定位下真空辅助乳腺病灶旋切术,其余22例行SCNB。结果 38例行立体定位真空辅助旋切患者,全部一次性切除成功;22例行SCNB患者中,漏诊乳腺癌1例,诊断特异性100.00%（13/13）,敏感性88.89%（8/9）。结论应用全数字化乳腺X光机及配套的立体定位活检系统,可有效提高对老年隐匿性乳腺病变的诊断能力,并可对微创手术治疗提供精确的导向作用。Objective To explore the diagnostic and therapeutic value of the vacuum assisted rotary cut concomitant with stereotactic core needle biopsy（SCNB）in the full digitalized mammography stereotaxis on elderly patients with recessive breast lesions.Methods A total of 60 elderly patients with recessive breast lesions who had negative clinical examinations and abnormal imageology in our hospital from Jan.2012 to Jun 2013 were selected,in which 38 with suspicious calcinosis were given vacuum assisted breast rotary cut,while the rest 22 were treated with SCNB.Results The 38 patients with vacuum assisted breast rotary cut were successfully incised the lesions primarily,whereas the 22 with SCNB had 1case of missed diagnosis of breast cancer with the diagnostic specificity and sensitivity being 100%（13/13）and 88.89%（8/9）respectively.Conclusion The application of full digitalized mammography stereotaxis X machine and matching SCNB can effectively improve the diagnostic ability of the recessive breast lesions in elderly patients,and provide an accurate guidance for minimally invasive surgeries.
