摘　　要：目的探讨肾恶性肿瘤患者经后腹腔镜肾动脉分支阻断、肾部分切除术后对肾功能的影响。方法收集2012年2月至2014年2月收治的、行后腹腔镜肾动脉分支阻断肾部分切除术的45例肾肿瘤患者的临床资料,对比术前、术后肾脏肾小球滤过率及血肌酐水平,分析手术时间、肾动脉阻断时间、肿瘤大小等各项指标,观察复发及转移情况。结果 45例患者的手术时间为（119.6±25.33）min,肾动脉阻断分支时间为（25.67±9.35）min,肿瘤直径为（3.7±0.8）cm。全组患者术前与术后血肌酐含量差异无统计学意义（t=1.90,P〉0.05）。全组患者术前与术后肾小球滤过率差异有统计学意义（t=3.98,P〈0.01）。1例患者于术中行输血治疗,于术后5d将腹膜外引流管拔出。全部患者于术后随访3~6个月,未出现肿瘤复发或远处转移。结论肾恶性肿瘤经后腹腔镜肾动脉分支阻断肾部分切除术治疗后,可取得显著的临床疗效,肾功能恢复情况较好,值得推广。Objective To explore and analyze kidney cancer after laparoscopic renal artery branches blocking the body part on renal function after resection.Methods The clinical data of Linqu People＇s Hospital of Shandong Province laparoscopic renal artery branches blocking partial nephrectomy since February 2012 to February 2014 were treated 45 cases of kidney cancer patients,before comparing preoperative and postoperative renal glomerular clinical data filtration rate and serum creatinine.The operation time for the group of patients,the indicators of renal artery occlusion time,tumor size and other statistics.After the short-term follow-up study from March to June,to observe whether recurrence and metastasis.Results The group of 45 patients,mean operative time was（ 119.6 ± 25.33） min,renal artery branches blocking the average time was（ 25.67 ± 9.35） min,mean tumor diameter in the range of（ 3.7 ± 0.8） cm.The group had no preoperative and postoperative serum creatinine levels compared to a significant difference（ t = 1.90,P〉 0.05）.The group of patients before surgery and postoperative glomerular filtration rate compared significant difference（ t = 3.98,P〈 0.01）.One patient in intraoperative blood transfusion therapy,accounting for 2.22%,within five days after the peritoneal drainage tube pulled.All patients were followed up at 3 ~6 months,does not appear tumor recurrence or distant metastasis.Conclusion Renal cancer after laparoscopic renal artery branches blocking partial nephrectomy after treatment can achieve significant clinical efficacy,recovery of renal function better,worthy of promotion.
