白介素-21在喉癌组织中的表达与意义

摘　　要：目的观察白介素(interleukin-21,IL-21)在喉癌(laryngeal carcinoma)肿瘤组织及癌旁周围正常组织中的表达,探讨炎症因子IL-21的表达与喉癌发病机制的关系。方法随机收集病理检测确诊为喉癌患者的喉癌组织及癌旁正常组织,记录患者一般情况和临床病理特征等资料。应用RT-PCR、Western blot方法检测IL-21在喉癌及癌旁正常组织中的基因和蛋白表达水平。然后采用免疫组化、免疫荧光等方法检测IL-21在喉癌及癌旁正常组织结构中的分布情况,观察其在组织中的表达特点,并结合临床参数探讨IL-21与喉鳞状细胞癌临床病理特征的相关性。结果 RT-PCR显示,与癌旁正常组织相比,喉癌组织IL-21 mRNA表达明显增高,差异具有统计学意义(P<0.01);Western blot显示,与癌旁正常组织相比,喉癌组织IL-21表达明显增高,差异具有统计学意义(P<0.01);IL-21表达与肿瘤的部分临床参数如肿瘤分化程度、淋巴结转移程度等因素密切相关。免疫组化显示IL-21呈微弱阳性表达于癌旁正常组织中。与此相比,喉癌组织中IL-21则呈现高表达;部分肿瘤细胞及炎性细胞在细胞核和细胞浆中均有高表达和分布。免疫荧光提示HLA-DR阳性的炎症细胞可能是IL-21因子分泌的重要来源。结论 IL-21在喉癌组织中基因水平和蛋白水平表达都明显增加,与临床进展密切相关,说明IL-21在喉癌病理形成过程中起到一定的作用,可能作为判断临床预后的重要指标。This study designed to observe the distribution and expression of IL-21（interleukin-21,IL-21） in laryngeal carcinoma tissue and adjacent normal tissue,and explore the role of IL-21 in the pathogenesis laryngeal carcinoma.Clinical data of 96 cases of pathologically diagnosed laryngeal carcinoma were collected in this study.RT-PCR and Western blot analysis showed that both gene and protein expression of IL-21 was significantly increased in laryngeal carcinoma,compared to adjacent normal tissues（P〈0.01）;the expression of IL-21 closely related to some the clinical parameters of the tumor.Immunohistochemistrical assay indicated that cells of adjacent normal tissue showed very weak expression of IL-21,while tumor cells and inflammatory cells in laryngeal carcinoma tissue showed significant strong expression of IL-21,and IL-21 was strongly expressed in the nucleus and cytoplasm in the laryngeal carcinoma.The increase of gene and protein expression of IL-21 in laryngeal carcinoma were closely associated with clinical progression,indicating that IL-21 plays a role in the development process of laryngeal pathology,thus IL-21 may be used as an important prognostic indicator in clinical judgment.
