术前置泵区域动脉灌注化疗治疗晚期乳腺癌远期疗效分析

摘　　要：目的比较术前区域动脉灌注化疗和全身静脉化疗对晚期乳腺癌远期疗效的影响。方法对2003年1月-2008年12月解放军309医院普外科97例经术前区域动脉灌注化疗和89例经术前全身静脉化疗的晚期乳腺癌患者进行长期随访,比较两组患者的局部复发、远处转移、总生存率(OS)和无瘤生存率(DFS),及两种治疗方法的远期疗效。结果术前区域动脉灌注化疗组和术前全身静脉化疗组患者术后远处转移率分别为15.5%和14.6%,差异无统计学意义(P>0.05),术后1年局部复发率分别为7.2%和18.0%,差异有统计学意义(P<0.01)。两组患者术后1年OS(分别为99.0%和96.6%)和DFS(分别为81.8%和68.5%)比较,差异均无统计学意义(P>0.05);术后3年OS两组之间差异无统计学意义(分别为88.7%和79.8%,P>0.05),但DFS之间差异有统计学意义(分别为70.1%和52.8%,P<0.05);术后5年两组患者OS和DFS之间差异均有统计学意义(P<0.01)。结论术前区域动脉灌注化疗与术前全身静脉化疗相比,可以减少晚期乳腺癌的局部复发率,提高5年OS和DFS。Objective To compare the long term therapeutic effects of the preoperative regional arterial infusion chemotherapy and the preoperative systemic venous chemotherapy in advanced breast cancer. Methods One hundred and eightysix patients with advanced breast cancer, admitted to the Department of General Surgery of PLA 309 Hospital from Jan. 2003 to Dec. 2008, were followed up for a long time. Of them 97 patients received preoperative regional arterial infusion chemotherapy（IAIC groups）, and 89 patients received preoperative systemic vein chemotherapy（IVIC groups）. The local recurrence rate, distant metastasis rate, overall survival rate（OS） and disease free survival rate（DFS） in the two groups were statistically analyzed, and the long term therapeutic effects of the two groups were compared. Results There were no statistically significant difference in distant metastasis rate（15.5% and14.6%）, 1-year OS（99.0% and 96.6%） and DFS（81.8% and 68.5%）, and 3-year OS（88.7% and 79.8%） between the two groups（P〈0.05）. While significant differences were found between the two groups（P〉0.01） in post-operative local recurrence rate（7.2% and 18.0%）, 3-year DFS（70.1% and 52.8%）, and 5-year OS and DFS. Conclusion Compared with preoperative systemic venous chemotherapy, pre-operative regional arterial infusion chemotherapy can decrease the local recurrence rate, improve the 5-year total survival rate and disease free survival rate for advanced breast cancer.
